Treatment and prevention of recurrence of gastric and duodenal ulcers; treatment of esophagitis and heartburn; improvement of gastric lesions
This medication is effective not only for the treatment of active gastric and duodenal ulcers but also for preventing the recurrence of duodenal ulcers. It is also used to treat heartburn symptoms due to endoscopically diagnosed erosive and ulcerative esophagitis and gastroesophageal reflux disease. It helps improve gastric mucosal lesions such as erosion, hemorrhage, redness, and edema that occur during acute exacerbations of acute and chronic gastritis. In summary, this drug is used for the treatment of gastric and duodenal ulcers, symptom relief from esophagitis and reflux disease, and improvement of gastric mucosal health.
1. Treatment of active gastric and duodenal ulcers Adults: Administer 300mg of nizatidine once daily at bedtime or 150mg orally twice daily, in the morning and evening. Most ulcers heal within 4 weeks, but treatment may be continued for up to 8 weeks if necessary. 2. Prevention of duodenal ulcer recurrence Adults: After healing of a duodenal ulcer, administer 150mg orally once daily at bedtime. Treatment may continue for up to 1 year; results of longer-term administration are not known. 3. Gastroesophageal reflux disease Adults: Administer 150mg orally twice daily, in the morning and evening. Treatment may continue for up to 12 weeks. 4. Improvement of gastric mucosal lesions Adults: Administer 150mg orally twice daily, in the morning and evening, for 2 weeks. 5. Dose adjustment for patients with severe renal impairment The dose for patients with renal impairment should be reduced as follows. Creatinine clearance Dose Active duodenal ulcer Gastroesophageal reflux disease (GERD) Benign gastric ulcer 20~50 ml/min 150mg daily < 20 ml/min 150mg every other day Maintenance therapy 20~50 ml/min 150mg every other day < 20 ml/min 150mg every 3 days Some older adults may have a creatinine clearance below 50mL/min; based on pharmacokinetic data in patients with renal impairment, the dose should also be reduced in these patients according to the degree of renal impairment. The clinical impact of this dose reduction in patients with renal impairment has not been evaluated.
This medication should not be administered to certain patients. In particular, it should not be given to patients who have had allergic reactions to this drug or other H2-receptor antagonists. It should also be avoided in patients with lactose intolerance or related genetic disorders due to the presence of lactose. Caution is necessary when administering this medication to some patients. Patients with a history of drug allergies, impaired liver function, and decreased kidney function require special attention. In particular, the dosage should be reduced or the dosing interval extended in patients with impaired kidney function. Rare allergic reactions such as skin rashes, itching, breathing difficulties, and swelling of the face or neck may occur when taking this medication, and if such symptoms arise, use should be stopped immediately. Blood-related side effects have been reported, including anemia, changes in white blood cell counts, and thrombocytopenia, and if these symptoms occur, the medication should be discontinued and a healthcare professional consulted. Gastrointestinal symptoms such as constipation, diarrhea, dry mouth, and nausea may occur. Liver function abnormalities have also been reported, and caution is required as liver enzyme levels may rise or jaundice symptoms may manifest. Neurological side effects may rarely include confusion, headaches, drowsiness, dizziness, and, in severe cases, seizures, so caution is warranted. Cardiac-related side effects may manifest as asymptomatic ventricular tachycardia. Hormonal-related side effects such as gynecomastia in women have been reported infrequently, as well as erectile dysfunction or decreased libido, but these occur at a general level. Skin-related side effects may include sweating, hives, rashes, dermatitis, and vasculitis. Other issues reported include hyperuricemia unrelated to gout, fever, chest pain, muscle pain, rhinitis, pharyngitis, and cough. This medication may conceal the symptoms of gastric cancer, so it should only be used after confirming that it is not cancer. Since it is excreted through the kidneys, dosage adjustments should be made for patients with moderate to severe impaired kidney function. During treatment, regular blood tests and liver and kidney function tests should be conducted, and if ineffective, switching to another medication is advisable. The safety of this medication in pregnant or breastfeeding women has not been established, so it should only be administered when absolutely necessary, as it may affect the fetus or newborn. The safety for pediatric use has not been established, and elderly patients may have a higher likelihood of decreased kidney function, so kidney function tests should be performed before administration, and careful monitoring should continue during treatment. This medication tends to have fewer drug interactions; however, taking it with aspirin in high doses may increase serum salicylate levels. In cases of overdose, symptoms such as vomiting, diarrhea, increased salivation, and pupil constriction may occur, and treatment should include clinical examination and supportive care, considering the use of activated charcoal or gastric lavage if necessary. It is difficult to effectively remove this medication through hemodialysis. Although there is no evidence of carcinogenicity with long-term high-dose use, some animal studies have shown changes in the liver and gastric mucosa, so caution is necessary with prolonged usage. In summary, this medication requires careful monitoring of kidney and liver function, and if any allergic reactions or abnormal symptoms occur, immediate consultation with medical personnel is essential.
Not everyone experiences these, and they can vary from person to person.
