Monotherapy and adjunctive therapy for partial-onset seizures
Who takes it
Adults · Children
Adult dose
Because this medicine is an extended-release tablet, swallow it whole without crushing, splitting, or chewing. This medicine may be taken with or without food, with plenty of water. Take the daily dose once daily. Monotherapy and adjunctive therapy for partial-onset seizures (12 years and older) Adults (18 years and older) Start this medicine at 1,000mg once daily. Depending on clinical response and tolerability, the dose may be increased or decreased by 1,000mg once daily every 2 weeks. The maximum recommended dose is 3,000mg per day. Dose adjustment in adults with renal impairment Normal, creatinine clearance ≧ 80 (mL/min/1.73m2): Single dose 1,000-3,000mg, once daily (24-hour interval) Mild, creatinine clearance 50-79 (mL/min/1.73m2): Single dose 1,000-2,000mg, once daily (24-hour interval) Moderate, creatinine clearance 30-49 (mL/min/1.73m2): Single dose 500-1,500mg, once daily (24-hour interval) Severe, creatinine clearance < 30 (mL/min/1.73m2): Single dose 500-1,000mg, once daily (24-hour interval) No dose adjustment is necessary in patients with mild to moderate hepatic impairment. In patients with severe hepatic impairment, creatinine clearance may underestimate renal impairment. Therefore, when creatinine clearance is less than 60mL/min/1.73m2, reducing the daily maintenance dose by 50% is recommended. Older adults (65 years and older) Dose adjustment is recommended in older patients with impaired renal function.

Precautions

This medication is used for the treatment of epilepsy and carries a risk of mental health issues such as suicidal thoughts or behaviors, and depression while taking it. Therefore, patients and caregivers should immediately inform healthcare professionals if any of these symptoms arise. This medication may affect the central nervous system, causing drowsiness, fatigue, coordination issues, and behavioral changes, especially during initial use. Both children and elderly patients may experience similar symptoms. Abruptly stopping the medication may worsen seizures, so the dosage must be gradually reduced under the guidance of healthcare professionals. Patients with a hypersensitivity to the components of this medication should not take it, and dosage adjustments may be necessary for those with kidney or liver dysfunction. It is recommended to conduct regular blood tests to monitor blood cell counts while taking this medication, and patients with heart disease or electrolyte imbalances should be carefully monitored with tests such as an electrocardiogram. If any unusual behaviors, aggression, anxiety, or depressive psychiatric symptoms occur during treatment, one should consult healthcare professionals immediately. Women who are pregnant or planning to become pregnant should consult healthcare professionals before taking this medication, and it is not recommended for breastfeeding. Care should be taken when performing hazardous tasks such as driving or operating machinery, especially during the early stages of treatment or when dosage changes are made due to potential drowsiness. In case of overdose, immediately visit a medical facility for appropriate treatment, as there is no specific antidote, and symptomatic treatment may be necessary. This medication has minimal interactions with other drugs, but caution is advised when used with certain medications, so patients should inform their healthcare providers about any current medications. Children, the elderly, and patients with decreased renal or liver function require careful attention and dosage adjustments during treatment. Keep the medication in a safe place out of the reach of children, and avoid transferring it to another container, as this could pose a risk of accidents and quality degradation.
