Treatment of respiratory, skin, and mycobacterial infections and eradication of Helicobacter pylori in duodenal ulcers
This medication is effective against various bacteria and is primarily used to treat lower respiratory tract infections such as bronchitis or pneumonia, upper respiratory tract infections including pharyngitis or sinusitis, as well as skin and soft tissue infections. It also aids in the treatment of duodenal ulcers by eradicating Helicobacter pylori bacteria. It is effective against mycobacterial infections, including Mycobacterium avium and intracellular infections. In summary, this drug is used for treating respiratory and skin infections as well as specific bacterial infections.
Adults: The usual dose of clarithromycin is 250mg (potency) twice daily. For severe infections, administer 500mg (potency) twice daily. The usual duration of treatment is 7~14 days. In patients with renal failure whose creatinine clearance is 30mL/min or less, the dose should be reduced by half, i.e., 250mg (potency) once daily or, for severe infections, 250mg (potency) twice daily. Do not administer this medicine to such patients for more than 14 days. Mycobacterial infections: Administer 500mg (potency) of this medicine twice daily. In this case, it should be administered in combination with other antimycobacterial agents, such as ethambutol, clofazimine, and rifampin. Eradication of Helicobacter pylori in patients with duodenal ulcer: Administer 500mg (potency) of this medicine 3 times daily in combination with omeprazole 40mg once daily for 14 days, followed by omeprazole 40mg once daily for 14 consecutive days. That is, administer clarithromycin 500mg (potency) 3 times daily for 14 days (days 1~14) and omeprazole 40mg once daily for 28 days (days 1~28).
Children aged 12 years and older: Same as adults. For children under 12 years of age, use clarithromycin dry syrup.
This medication should not be used in specific patients. For example, patients allergic to any components in this medication or who have had adverse reactions to similar classes of antibiotics should avoid it. Additionally, it should not be administered to patients currently taking certain heart-related medications or those with a history of prolonged QT interval cardiac disease, severe liver impairments, or significantly reduced kidney function. Pregnant or nursing women and patients with central nervous system infections should avoid using this medication for safety reasons. This drug may affect liver and kidney function; hence, it should be used cautiously in patients with liver dysfunction or renal failure, especially among the elderly or patients with heart disease. Since there can be allergic reactions or severe side effects while using this drug, if symptoms such as shortness of breath, rash, severe skin abnormalities, gastrointestinal distress, or neurological symptoms occur, administration should be stopped immediately and consult a healthcare professional. This medication has many interactions with other drugs, particularly with those affecting the heart, medications metabolized by the liver, hypoglycemic agents, anticoagulants, and certain antibiotics, so it's important to inform a doctor or pharmacist when using them, as dose adjustments or monitoring may be necessary. Pregnant or nursing mothers should avoid this medication; if necessary, breastfeeding should be discontinued. Use in children under 12 is not recommended in tablet form, and caution is needed even in pediatric cases. Elderly patients may have increased drug concentrations due to decreased bodily function, and care should be taken in their administration; dose adjustment is required for severely reduced kidney function. Regular liver function tests, kidney function tests, and blood tests are required to detect any adverse reactions early and respond accordingly. In case of overdose, gastrointestinal disturbances or neurological symptoms may appear, thus the exact dosage should be followed as directed by a physician. This medication may have reduced absorption rates when used with specific preparations, so be cautious about the medications taken concurrently. For storage, keep in a tight container that blocks light and maintain a temperature between 15 degrees and 30 degrees Celsius. This will help preserve the drug's effectiveness and safety.
