Reduction in platelet count, reduction in the risk of thrombosis, and improvement of symptoms of thrombosis and bleeding
This medication is used to lower the elevated platelet levels caused by myeloproliferative disorders, reduce the risk of thrombus formation, and improve related symptoms such as thrombosis and bleeding tendencies. Therefore, it can effectively manage symptoms caused by thrombocytosis.
In adults, the starting dose is 0.5mg 4 times daily or 1.0mg 2 times daily, administered with close monitoring and maintained for at least 1 week. In both adults and children, the dose should be the lowest effective dose necessary to reduce the platelet count to 600,000/μL or less, while keeping the platelet count within the normal range as much as possible. If the dose is increased, the amount of the increase should be no more than 0.5mg/day, and the dose should not be increased again within 1 week of that increase. The maintenance dose is the same for adult and pediatric patients. Do not exceed 2.5mg as a single dose or 10mg as the total daily dose. To monitor the effect of this medicine and prevent the onset of thrombocytopenia, check the platelet count every 2 days during the first week of treatment and at least once weekly after reaching the maintenance dose. In general, the platelet count begins to decrease within 7~14 days at the appropriate dose. It takes 4~12 weeks to achieve a complete response, defined as a platelet count of 600,000/μL or less. Most patients will show an adequate response with a dose of 1.5~3.0mg/day.
The starting dose for children ranges from 0.5 mg once daily to 0.5 mg 4 times daily; based on limited data, an initial dose of 0.5 mg once daily is recommended for children.
This drug should not be used in patients with severe liver dysfunction, as it may significantly increase the drug concentration in patients with moderate to severe impairment of liver function. It should also not be administered to patients with genetic disorders impacting lactose digestion due to the lactose contained in this drug. Caution is necessary when using this drug in patients with suspected or existing heart disease, as it may increase heart rate and enhance cardiac contractility, potentially placing strain on the heart. Even in patients with normal cardiac health, heart-related side effects may occur, hence cardiac evaluations should be conducted before and during treatment to monitor for these effects. Patients with mild to moderate liver impairment require dosage adjustments and careful monitoring of liver function and cardiovascular status. Side effects related to interstitial lung disease or pulmonary hypertension have been reported, necessitating assessment of respiratory symptoms before and during treatment. Common side effects during treatment may include headache, palpitations, edema, nausea, diarrhea, dizziness, depression, memory loss, confusion, insomnia, paresthesia, sensory loss, neurosis, dry mouth, migraine, stuttering, drowsiness, abnormal coordination, diplopia, visual disturbances, tinnitus, increased blood pressure, nausea, gastrointestinal disorders, loss of appetite, dyspepsia, constipation, gastrointestinal diseases, rash, alopecia, pruritus, skin discoloration, dry skin, arthralgia, myalgia, lumbar pain, erectile dysfunction, nocturia. Platelet levels can fluctuate quickly upon discontinuation of this drug, thus, platelet levels should be monitored frequently. Sudden cessation of the drug may lead to severe complications such as thrombosis and should be avoided. Regular blood tests, liver function tests, kidney function tests, and electrolyte tests should be conducted during treatment to confirm any abnormal findings. Blood pressure may drop, so be mindful of symptoms such as dizziness. When combining with other medications that increase the risk of bleeding, such as aspirin, careful evaluation of the risks and benefits before concurrent use is necessary. This drug should not be administered to pregnant women or those who could become pregnant during treatment. Women are advised to avoid pregnancy while using this medication. Breastfeeding women should consider discontinuing either breastfeeding or the medication, as it may be secreted in breast milk. No special dosage adjustments are needed when treating children or elderly patients, though elderly patients may be at higher risk for heart-related side effects and should be monitored closely. In cases of overdose, a reduction in platelet count and cardiovascular and central nervous system toxicities may occur, necessitating immediate medical supervision. This medication should be kept out of reach of children, and caution is needed when transferring to other containers as this may affect its quality.
