Summary

Main use
Reduction in platelet count, reduced risk of thrombosis, and improvement of symptoms of thrombosis and bleeding
Who takes it
Adults · Children
Adult dose
For adults, the starting dose is 0.5mg 4 times daily or 1.0mg 2 times daily, administered under close monitoring and maintained for at least 1 week. In both adults and children, the dose should be the lowest effective dose required to reduce the platelet count and maintain it at 600,000/μL or below, while keeping the platelet count within the normal range as far as possible. If the dose is increased, the increment should be no more than 0.5mg/day, and the increased dose should not be increased again within 1 week. The maintenance dose is the same for adult and pediatric patients. Do not exceed 2.5mg as a single dose or 10mg as the total daily dose. To monitor the effect of this medicine and prevent the development of thrombocytopenia, monitor the platelet count every 2 days during the first week of treatment and at least once weekly after the maintenance dose is reached. In general, the platelet count begins to decrease within 7~14 days at an appropriate dose. It takes 4~12 weeks to achieve a complete response, defined as a platelet count of 600,000/μL or below. Most patients will show an adequate response at a dose of 1.5~3.0mg/day. Patients with a history of heart disease, renal failure, or hepatic impairment, or in whom these conditions are suspected, should be carefully monitored.
Child dose
The starting dose for children is in the range of 0.5mg/day to 0.5mg 4 times daily. Based on limited data, an initial dose of 0.5mg/day is recommended for children.

This medication is used for patients with elevated platelet counts due to myeloproliferative disorders. It helps improve the symptoms of thrombocytosis seen in various myeloproliferative disorders such as essential thrombocythemia, polycythemia vera, and chronic myeloid leukemia. This medication effectively lowers platelet counts to reduce the risk of thrombosis. Additionally, it alleviates symptoms related to the simultaneous occurrence of thrombosis and bleeding, thereby lessening discomfort for patients. Consequently, it plays an important role in promoting smooth blood circulation and preventing complications. This medication improves symptoms associated with thrombocytosis.

Precautions

This medication should not be used in patients with severe liver dysfunction, as it can significantly increase the concentration of the medication in the body when liver function is moderately impaired or worse. Moreover, patients with lactose intolerance or certain genetic issues should not take this medication due to the lactose content. Caution is necessary in patients with suspected or existing heart conditions, as this medication may increase heart rate and contractility, putting stress on the cardiovascular system. Cardiac function tests should be conducted before and during treatment, and continuous monitoring is required. Patients with mild liver impairment should also be cautious when taking this medication, and dosage adjustments may be necessary based on liver function test results. This medication may cause symptoms related to interstitial lung disease or pulmonary hypertension, so if respiratory symptoms occur, immediate consultation with healthcare professionals is necessary. During treatment, side effects such as headaches, palpitations, edema, nausea, and diarrhea may occur, and if these symptoms persist or worsen, patients should notify their healthcare providers. Due to the potential for rapid changes in platelet count, regular blood tests are required, and abrupt discontinuation should be avoided due to the risk of sudden increases in platelet count and thrombosis. This medication may interact with other drugs, so especially if taking aspirin or heart-related medications, patients should consult their physician. Women who are pregnant or may become pregnant should not take this medication, and if pregnancy is confirmed during treatment, they should notify their healthcare provider immediately. Breastfeeding women should also carefully consider the use of this medication, as it may be secreted in breast milk and potentially affect the infant. Caution is also necessary in pediatric and elderly patients, and if any adverse reactions occur, immediate consultation with healthcare providers should be sought. In cases of overdose, reduced platelet counts and cardiovascular symptoms may occur, so accurate dosing as per the physician's instructions is crucial, and immediate treatment should be sought if abnormal symptoms develop. This medication should be kept out of the reach of children, and care should be taken to prevent quality degradation by transferring it to other containers.
