Precautions

This medication should not be used in certain patients. For example, patients allergic to this medication or its ingredients, patients with cardiogenic shock, patients with severe cardiac conduction abnormalities, patients with pulmonary heart disease, patients with diabetic ketoacidosis, patients with severely impaired cardiac function, patients with asthma or chronic obstructive pulmonary disease, patients with secondary hypertension, patients with Prinzmetal angina, patients with acute pulmonary embolism, patients with pheochromocytoma, patients taking certain medications, patients with severe hypotension, patients with severe hepatic impairment, women who are pregnant or breastfeeding, patients with peripheral circulation disorders, patients with laryngeal edema, patients with complications from myocardial infarction, patients with fluid overload, patients with allergic rhinitis, and patients with lactose intolerance should not receive this medication. Caution is also advised for patients with diabetes, those with a history of hypersensitivity or those receiving sensitive treatment, patients with a history or family history of psoriasis due to beta-blockers, patients with cardiovascular diseases or renal failure with accompanying hypotension, patients with unstable angina, patients with chronic heart failure, patients with severe renal impairment, patients with first-degree heart block, patients with mild to moderate bradycardia, patients using beta-blockers in eye drop form, elderly patients, patients with blood pressure drop associated with position changes, patients with acute inflammatory heart disease, patients taking certain receptor agonists concurrently, patients with sodium or fluid depletion, patients concurrently taking cardiac glycosides or specific calcium channel blockers, fasting patients, patients with hyperthyroidism, contact lens users, children and adolescents under 18 years of age, and hemodynamically unstable patients with acute myocardial infarction, among others. Dizziness, headache, fatigue, and other symptoms may occur when taking this medication, and psychiatric-related symptoms have been reported rarely. Serious side effects such as decreased heart rate, decreased blood pressure, and worsening heart failure may occur, requiring regular cardiac function monitoring. Furthermore, various side effects may occur, including renal function deterioration, increased respiratory resistance, gastrointestinal symptoms, skin rashes, blood abnormalities, liver function abnormalities, and endocrine changes. Long-term administration requires regular testing of heart function, blood pressure, electrocardiograms, and liver and kidney function, and sudden cessation of this medication may worsen symptoms or trigger a myocardial infarction; therefore, it should be gradually reduced as directed by a physician. Caution is also needed when driving or operating dangerous machinery, especially for elderly patients or those with impaired kidney and liver function. This medication can interact with various drugs, thus caution is needed when used concomitantly with other antihypertensive agents, sedatives, antidepressants, calcium channel blockers, insulin, antiarrhythmics, NSAIDs, and alcohol. Additionally, monitoring of concomitant medications is important as it may alter the blood levels of some drugs. This medication should not be administered to pregnant or possibly pregnant women and breastfeeding mothers, as it may cause side effects in fetuses and newborns. Safety for use in children and adolescents has not been established, and its use is not recommended, especially in young children with severe heart failure, which may present severe hypoglycemia symptoms. Elderly patients may have an increased blood concentration of this medication due to decreased liver function, and they should be started on a low dose due to the risk of excessive blood pressure reduction leading to risks such as stroke. In case of overdose, symptoms may include decreased heart rate, reduced cardiac function, shock, respiratory distress, and altered consciousness; please notify medical personnel immediately and seek appropriate treatment, as this medication is not removed by blood dialysis and requires detoxification and supportive therapy. This medication is to be used only in heart failure patients with a low left ventricular ejection fraction and in a stable state, and when increasing the dosage, careful monitoring for worsening heart failure symptoms is required. If symptoms worsen, dosage adjustment of diuretics or drug reduction/discontinuation may be necessary. Finally, this medication may discolor due to light exposure, so it should be stored in an opaque container and kept out of reach of children; transferring to other containers is harmful to quality maintenance.
