This medication is effective against various harmful bacteria such as Streptococcus, Staphylococcus, Clostridium difficile, and diphtheroid bacteria. It is used for the treatment of endocarditis, osteomyelitis, arthritis, peritonitis, meningitis, superficial secondary infections that may occur after burns and surgery. It is also effective for serious lung and systemic infections such as pneumonia, sepsis, lung abscess, and empyema. It is also used for the treatment of severe infections that are difficult to treat with penicillin or cephalosporin antibiotics. When taken orally, it helps treat antibiotic-associated pseudomembranous colitis caused by Clostridium difficile. Overall, this drug is effectively used for the treatment of various bacterial infections.

Dosage and Administration

Adult dosage

As vancomycin, administer 500 mg (potency) intravenously every 6 hours or 1 g (potency) every 12 hours. For oral administration, take 500-2000 mg over 3-4 divided doses daily for 7-10 days, not exceeding a total daily dose of 2 g.

Children's dosage

Administer 10 mg/kg (potency) intravenously every 6 hours. Infants should receive 10-15 mg/kg (potency) every 12 hours. Infants and newborns are administerly given an initial dose of 15 mg/kg.

Precautions

This medication should not be used in patients who have previously experienced severe allergic reactions or shock due to this drug. It is also safest not to administer this drug to patients who are allergic to it or to peptide- and aminoglycoside-based antibiotics. Generally, it should be avoided in patients with hearing loss or deafness, but should be used carefully if absolutely necessary. For patients with kidney function issues or acute urinary retention symptoms, this drug can accumulate in the body and cause side effects, so blood concentrations should be frequently monitored, and administration should be done carefully. Serious allergic reactions such as shock or anaphylaxis can occur rarely when using this medication, so patients should be closely monitored, and administration should be stopped immediately if any unusual symptoms occur. Gastrointestinal issues may rarely occur, including diarrhea, nausea, vomiting, and abdominal pain. Hematological issues, such as decreased red blood cell, white blood cell, platelet counts, or increased eosinophils, can appear, so regular blood tests are advisable. In the central nervous system, dizziness, tinnitus, or hearing loss may occur, so hearing tests should be conducted to monitor the condition. Routine liver function tests should also be performed, and actions should be taken if abnormalities are found. Kidney function tests are necessary, and if issues are detected, dosage should be adjusted or stopped. Severe skin reactions such as rashes or blistering dermatitis, Stevens-Johnson syndrome can occur on the skin, so caution is warranted. Rapid infusion can lead to rash and pain on the face, neck, and torso, as well as muscle spasms, thus it should be administered slowly over a minimum of 60 minutes. Long-term use of this drug can lead to the development of resistant bacteria, so it is important to administer it only for the necessary duration. If diarrhea, abdominal pain, or fever persists after the use of antibiotics, pseudomembranous colitis should be suspected, and immediate consultation with a physician is necessary. Additionally, caution is necessary when using this medication alongside others that may affect the kidneys or nerves, as it can increase the risk of side effects. Pregnant or breastfeeding women should only use this medication when absolutely necessary, as it may affect the fetus or baby, and only under the guidance of a specialist. Newborns or premature infants should be administered carefully as their kidneys may not fully develop, which can prolong drug retention in the body. The elderly are likely to have impaired kidney function, so kidney function tests should be performed before and during administration, and dosage should be adjusted as needed. In case of overdose, severe side effects such as acute renal failure or hearing loss can occur, necessitating immediate emergency treatment. This medication must be administered intravenously, as intramuscular administration or other routes can cause tissue damage. If the infusion rate is too fast, it can cause hypotension or heart problems, so it should be administered slowly.
