Prevention of cardiovascular events in coronary artery disease and prevention of stroke in patients with stroke or transient ischemic attack
This medication helps reduce the incidence of thrombotic cardiovascular events when used in conjunction with aspirin. It is particularly useful for patients with acute coronary syndrome or patients at risk of thrombotic cardiovascular events who have experienced a myocardial infarction more than one year ago. Additionally, it can lower the risk of myocardial infarction and stroke in patients with type 2 diabetes who have undergone percutaneous coronary intervention (PCI) for coronary artery disease. In cases of acute ischemic stroke or high-risk transient ischemic attacks (TIA), it reduces the incidence of stroke when used in combination with aspirin. Thus, this medication is effectively used for the prevention of thrombotic events in various cardiovascular disease patients.
Do not use this product concomitantly with other ADP receptor inhibitors (ticlopidine, clopidogrel, prasugrel). Acute coronary syndrome: Administer a 180mg loading dose, followed by 90mg per dose twice daily. Treatment is recommended for 12 months unless discontinuation of this product is clinically necessary. Patients taking this product should also receive a maintenance dose of aspirin (75 - 150mg), unless specifically contraindicated. In patients with acute coronary syndrome who have an increased risk of bleeding and have undergone percutaneous coronary intervention (PCI), monotherapy with this product (discontinuation of aspirin) may be considered after 3 months of concomitant treatment with this product and aspirin. Patients with a history of myocardial infarction (occurring at least 1 year previously) who are at high risk of thrombotic cardiovascular events: Administer 60mg per dose twice daily. Patients taking this product should also receive a maintenance dose of aspirin (75 - 150mg), unless specifically contraindicated. Patients who took 90mg of this product or another ADP receptor inhibitor (ticlopidine, clopidogrel, prasugrel) during the first year after myocardial infarction may continue treatment with 60mg of this product. Treatment with this product may also be initiated in patients within 2 years after myocardial infarction or within 1 year after discontinuing a previous ADP receptor inhibitor (ticlopidine, clopidogrel, prasugrel). The safety and efficacy of this product were evaluated at 3 years of follow-up. Patients with type 2 diabetes who have undergone percutaneous coronary intervention (PCI) for coronary artery disease: Administer 60mg per dose twice daily. Patients taking this product should also receive a maintenance dose of aspirin (75 - 150mg), unless specifically contraindicated. Refer to the dosage and administration for patients with acute coronary syndrome. Data on the safety and efficacy of this product for treatment lasting more than 3 years are limited. Patients with acute ischemic stroke or high-risk transient ischemic attack: Administer a 180mg loading dose, followed by 90mg per dose twice daily for 30 days. Patients taking this product should also receive a maintenance dose of aspirin (75 - 100mg), unless specifically contraindicated. An initial loading dose of aspirin (300-325mg) is recommended in patients with acute ischemic stroke or high-risk transient ischemic attack. This product is administered orally and may be taken with or without food. For patients who have difficulty swallowing tablets, crush this product into a fine powder, disperse it in half a glass of water, and drink the mixture immediately. Rinse the glass used with half a glass of water and drink again. This mixture may also be administered through a nasogastric tube (CH8 or larger); it is important to flush the nasogastric tube with water after administration. When switching from another antiplatelet agent to this product, start this product 24 hours after the last dose of the previous medication. If a dose is missed, take only 1 tablet (the next scheduled dose) at the next scheduled dosing time. No dose adjustment is necessary in patients with renal impairment. No dose adjustment is necessary in patients with mild hepatic impairment. This product has not been studied in patients with severe hepatic impairment, and there is limited treatment experience in patients with moderate hepatic impairment.
The safety and efficacy of this product in pediatric patients under 18 years of age have not been established.
This medication is designed to prevent blood clotting; however, it may increase the risk of bleeding, and should not be used in patients with bleeding disorders or a history of intracranial hemorrhage. If surgery is planned, it should be discontinued at least 5 days before the procedure, and caution is advised due to potential bleeding during surgery. If bleeding symptoms occur, notify healthcare professionals immediately, and treatment for the bleeding should be initiated without discontinuing the medication if possible. Patients with hypersensitivity to this medication, severe liver dysfunction, or who are taking strong CYP3A4 inhibitors should not use this medication. Caution should be exercised if administered to patients at high risk of bleeding or those taking other medications that may cause bleeding, especially when used concurrently with warfarin. Patients at risk of bradycardia or decreased heart rate, as well as those with asthma or chronic obstructive pulmonary disease, should also use caution when taking this medication. Respiratory distress may occur, and discontinuation of treatment should be considered in such cases. Women who are pregnant or breastfeeding should consult with a specialist regarding the potential effects on the fetus or newborn. Additionally, this medication may interact with various other drugs, so patients should inform healthcare providers about any other medications being taken.
