Treatment of herpes zoster; treatment of genital herpes and prevention of recurrence
This medication is used to treat herpes zoster virus infections. It is also effective in treating genital herpes infections and in suppressing recurrent genital herpes. In other words, it helps manage conditions related to herpes zoster and genital herpes.
1. Treatment of herpes zoster Adults: Administer 250 mg of famciclovir orally per dose 3 times daily for 7 days. It is preferable to start treatment as soon as symptoms appear after infection. 2. Treatment of initial genital herpes infection Adults: Administer 250 mg of this medicine orally per dose 3 times daily for 5 days. It is preferable to start treatment as soon as symptoms appear after infection. 3. Treatment of acute recurrent genital herpes infection Adults: Administer 125 mg of this medicine orally per dose twice daily for 5 days. It is preferable to start treatment during the prodromal period or as soon as symptoms appear after infection. 4. Suppression of recurrent genital herpes Adults: Administer 250 mg of this medicine orally per dose twice daily. Discontinue treatment periodically every 6~12 months to observe whether the natural course of the disease has changed. 5. Patients with renal impairment: Because the clearance of this medicine is reduced in patients with renal impairment, use caution when adjusting the dose. The dosage and administration for these patients are as follows. 1) Treatment of herpes zoster and initial genital herpes infection: Creatinine clearance (mL/min/1.73㎡) 30~59: 250 mg per dose twice daily 10~29: 250 mg per dose once daily 2) Treatment of acute recurrent genital herpes infection: Creatinine clearance (mL/min/1.73㎡) 30~59: 125 mg per dose twice daily 10~29: 125 mg per dose once daily 3) Suppression of recurrent genital herpes infection: Creatinine clearance (mL/min/1.73㎡) 30~59: 250 mg per dose twice daily 10~29: 125 mg per dose twice daily 4) If only serum creatinine is known, creatinine clearance can be calculated using the following formulas. Men: {body weight (kg) × (140-age)}/ {72 × serum creatinine (mg/dL)} Women: 0.85 × the value calculated for men 5) Patients with renal impairment undergoing hemodialysis For patients undergoing hemodialysis, it is preferable to administer this medicine every 48 hours, taking the dialysis interval into account. Four hours of hemodialysis reduces the plasma concentration of this medicine by approximately 75%; therefore, administer this medicine immediately after dialysis. The recommended dose is 250 mg for patients with herpes zoster and 125 mg for patients with genital herpes. 6. Patients with hepatic impairment: No separate dose adjustment is necessary in patients with chronic compensated liver disease. There is insufficient treatment experience in patients with overt chronic decompensated liver disease; therefore, a clear recommended dose has not been established.
This medication should not be used in certain patients. It should not be taken by individuals who are allergic to this medication or its ingredients, including the active substance penciclovir. Additionally, it contains lactose, so it should not be administered to patients with lactose intolerance or hereditary metabolic disorders. Patients with impaired kidney function should use caution when taking this drug. Mild side effects such as headaches or nausea may occur, and in rare cases, a decrease in blood cell count or neurological symptoms such as dizziness or hallucinations may arise. Gastrointestinal issues such as nausea, vomiting, abdominal pain, and diarrhea have been reported, along with possible liver function abnormalities, skin rashes, and itching. Those with impaired kidney function may be at risk for acute renal failure, and dosage adjustments may be necessary. This medication does not cure genital herpes. Hence, sexual contact should be avoided during treatment to prevent the spread of infection, and safe sexual practices should be maintained. If symptoms such as dizziness, drowsiness, or confusion occur while taking this medication, one should avoid driving or operating machinery. Special dosage adjustments are generally not required for patients with mild to moderate liver impairment, but its effectiveness may be reduced in patients with severe liver impairment. While there is a low risk of interactions with other drugs, caution is needed when taken with certain medications, especially those affecting renal excretion, monitoring for potential toxicity is advised. Women who are pregnant or breastfeeding should carefully consider the risks and benefits of using this medication, and women of childbearing potential should exercise special caution. It is generally recommended that this medication not be administered to children under 18 years of age, as safety and efficacy have not been established. Elderly patients typically do not require dosage adjustments; however, dosage should be adjusted for those with decreased kidney function. In cases of overdose, appropriate treatment should be administered, and hemodialysis may reduce the concentration of the active substance in the blood. This medication should be stored in a cool, dry place and kept out of the reach of children. Care should be taken not to transfer it to another container, as this may affect quality and safety.
