Treatment of mood disorders (depression and premenstrual dysphoric disorder), obsessive-compulsive disorder, and bulimia nervosa
This medication shows effects in improving depressive state and various psychiatric symptoms in patients with depression within 5 to 6 weeks. It also helps reduce binge eating and purging behaviors in cases of anorexia nervosa. The efficacy in alleviating symptoms for patients experiencing obsessive-compulsive disorder has also been demonstrated. It is used for relieving emotional instability and physical symptoms due to premenstrual dysphoric disorder. The long-term usefulness of this medication should be evaluated regularly, but it overall plays a positive role in improving various symptoms related to mental health.
Adults 1. Depression 1) Initial treatment: The initial dose is 20 mg of fluoxetine hydrochloride orally once daily in the morning. In controlled clinical trials, doses of 20 ∼ 80 mg per day were administered. If no improvement is observed, an increase in the dose may be considered after several weeks. Doses exceeding 20 mg per day should be administered in 2 divided doses (morning and noon), and the maximum daily dose of 80 mg should not be exceeded. The onset of an adequate antidepressant effect may be delayed until 4 weeks or longer after starting treatment. 2) Maintenance/continuation/extended treatment: There are no definitive data on how long patients should be treated with this medicine; however, psychopharmacologists generally agree that acute episodes of depression require pharmacotherapy for several months or longer. It is not known whether the amount of antidepressant needed to relieve symptoms is the same as that needed to maintain or sustain remission. 2. Bulimia nervosa Administer 60 mg of this medicine per day. The daily dose should not exceed 80 mg. Measure electrolyte concentrations before starting treatment. 3. Obsessive-compulsive disorder Administer 20 ∼ 60 mg of this medicine per day. The daily dose should not exceed 80 mg. 4. Premenstrual dysphoric disorder Administer 20 mg of this medicine per day. After 6 months of treatment, reassess whether continued treatment is necessary. Patients with renal and/or hepatic impairment, patients taking multiple medicines, elderly patients, and patients with concomitant diseases: Administer a lower dose or reduce the frequency of administration. Adjust the dose as appropriate according to age and symptoms. Switching from MAO inhibitors used to treat psychiatric disorders Allow an interval of at least 14 days after discontinuing an MAO inhibitor used to treat psychiatric disorders before starting treatment with this medicine. Conversely, at least 5 weeks must elapse after discontinuing this medicine before starting an MAO inhibitor to treat a psychiatric disorder. Other MAO inhibitors, such as linezolid or methylene blue Patients receiving linezolid or intravenous methylene blue should not start treatment with this medicine because of the increased risk of serotonin syndrome. For patients requiring treatment for a more urgent psychiatric condition, including hospitalization or other interventional procedures, consider administering treatment. It may be necessary to urgently administer linezolid or intravenous methylene blue to a patient already receiving this medicine. If there is no alternative to linezolid or intravenous methylene blue and, for a particular patient, the benefits of treatment with linezolid or intravenous methylene blue are judged to outweigh the risk of serotonin syndrome, discontinue this medicine immediately and administer linezolid or intravenous methylene blue. Monitor the patient for symptoms of serotonin syndrome for 5 weeks after administration of linezolid or intravenous methylene blue, or until 24 hours after the last dose, whichever comes first. Treatment with this medicine may be resumed 24 hours after the last dose of linezolid or intravenous methylene blue. The risk associated with administration of methylene blue by non-intravenous routes (oral tablets or local injection) or with intravenous administration of this medicine at doses of 1 mg/kg or less is unclear. Nevertheless, clinicians should be aware of the possibility of emergency symptoms of serotonin syndrome with these uses.
This medication may increase the risk of suicidal thoughts or behavior in children, adolescents, and young adults with major depressive disorder or other psychiatric conditions, so caution should be exercised when using it. Patients aged 18 to 24 require careful monitoring by a physician during the initial treatment, and family or guardians should closely observe the patient's condition. This medication should not be used in certain patients, such as those with allergies to this drug or its excipients, those with severe renal impairment, and those currently taking MAO inhibitors. Patients with genetic issues such as galactose intolerance or lactose intolerance should also avoid this medication. Patients with a history of seizures require caution due to the risk of seizures, and if seizures occur or increase in frequency, the medication should be discontinued. Patients with suicidal tendencies or a history of mania should also be treated cautiously and consult their physician immediately if symptoms worsen. Patients with heart disease, liver or kidney problems should also be cautious with this medication. Diabetic patients should carefully monitor their blood sugar levels, as this medication may affect glucose control. Patients with elevated intraocular pressure or at risk of glaucoma should also be cautious. Common side effects may include headache, nausea, insomnia, fatigue, diarrhea, and there may rarely be allergic reactions or severe skin reactions. Discontinuation of treatment may lead to withdrawal symptoms such as dizziness, sensory disturbances, anxiety, nausea, and tremors; thus, the dosage should be reduced gradually under physician direction. This medication may interact with other drugs, so patients should inform their physician or pharmacist of any other medications being taken. Women who are pregnant or breastfeeding should consult their physician before using this medication, as it may affect the fetus or newborn. Safety in children and adolescents has not been established, so use should be avoided if possible. Older adults may be at risk of adverse reactions depending on other conditions or medications they are taking.
