Adjunctive treatment of partial-onset seizures in patients with epilepsy aged 16 years and older
This medication is used as an additional treatment for partial-onset seizures in patients with epilepsy aged 16 years and older, and may also be effective in cases accompanied by generalized seizures. It is primarily used to reduce the frequency of seizures in epilepsy and to alleviate symptoms.
This medicine should be taken twice daily. The recommended initial dose is 50mg twice daily; after 1 week, increase the dose to 100mg twice daily. Depending on clinical response and tolerability, the maintenance dose may be increased by 50mg per dose twice daily each week. The maximum recommended dose is 400mg per day (200mg twice daily). If treatment with this medicine is to be discontinued, gradual dose reduction is recommended (e.g., gradually reduce the daily dose by 200mg each week). This medicine may be taken with or without food. No dosage adjustment is required in patients with mild or moderate renal impairment (CLCR>30mL/min). A maximum daily dose of 300mg is recommended in patients with severe renal impairment (CLCR≤30mL/min) and in patients with end-stage renal disease. Since this medicine is removed from plasma by hemodialysis, an additional dose of up to 50% of the single dose is recommended immediately after hemodialysis in patients receiving hemodialysis. Caution is advised when treating patients with end-stage renal disease because clinical experience is limited and metabolites (whose pharmacologic activity is unknown) may accumulate. Dosage adjustment should be made cautiously in all patients with renal impairment. Use caution when determining the dose in patients with hepatic impairment. A maximum daily dose of 300mg is recommended in patients with mild or moderate hepatic impairment. Use is not recommended in patients with severe hepatic impairment. Dose reduction is not required in elderly patients. Experience with lacosamide use in elderly patients with epilepsy is limited; however, reduced renal clearance and increased AUC values are associated with age and should be taken into consideration in elderly patients. (See section ‘Patients with renal impairment’)
There are no data on safety and efficacy in children and adolescents under 16 years of age; therefore, use in these age groups is not recommended.
This medication may increase the risk of suicidal thoughts or behavior, so if any mood or behavioral changes occur during treatment, you should consult a doctor immediately. Do not use this medication if you are allergic to any of its components. Dizziness may occur, so caution should be exercised to prevent falls or accidents when taking it for the first time; it should be administered under medical supervision if necessary. Patients with heart disease or conduction abnormalities should use this drug cautiously, and an electrocardiogram is recommended before administration. This medication may induce psychiatric symptoms such as depression, mood changes, or suicidal thoughts, so patients and guardians should closely monitor for these symptoms and inform medical staff immediately if any are observed. Severe allergic reactions called multiorgan hypersensitivity can occur rarely, so if symptoms such as fever or rash appear, the medication should be discontinued immediately and medical attention sought. Common side effects may include dizziness, headache, nausea, diplopia, depression, confusion, insomnia, mood changes, cognitive impairment, eye twitching, balance disorders, coordination disorders, memory impairment, tremors, drowsiness, speech disorders, attention deficits, sensory impairment, sensory abnormalities, cerebellar syndrome, blurred vision, vertigo, tinnitus, vomiting, constipation, abdominal distension, indigestion, dry mouth, diarrhea, muscle cramps, itching, gait disturbances, weakness, fatigue, and feelings of drunkenness, fever, and falls. It is advisable to avoid driving or operating dangerous machinery until you are accustomed to the effects of this medication. This medication may interact with other drugs, so you must inform your doctor or pharmacist of any medications you are currently taking, especially when taking heart-related medications or antiepileptics. Elderly patients generally do not require dosage adjustments, but caution may be necessary depending on their physical condition. The safety and efficacy of this medication have not been established in children or adolescents under 16 years of age, so its use in this age group is not recommended. This medication should be avoided during pregnancy unless specifically indicated, as it may affect the fetus, and breastfeeding should also be discontinued as the medication can be transmitted through breast milk. In cases of overdose, symptoms may include dizziness, nausea, and vomiting, and since no specific antidote exists, treatment based on symptoms is necessary. This medication has the potential to cause psychological dependence, so it should be taken according to a doctor’s instructions and not be discontinued or dosages altered arbitrarily.
