View approved original text

This medication helps adults with type 2 diabetes, particularly those who are overweight, when blood sugar control is insufficient with diet and exercise alone. It can be used as monotherapy, or in combination with other oral hypoglycemic agents or insulin to enhance blood sugar control. This contributes to stable management of blood sugar levels and helps reduce the occurrence of complications.

How to take

Adult dose

The initial dose is 500mg once daily with the evening meal. Blood sugar levels should be checked one week later to adjust the dosage. For dosage increases, it should be increased by 500mg weekly, taken with the evening meal, up to a maximum of 2000mg daily. If blood sugar control is insufficient at 2000mg once daily, consider taking 1000mg twice daily with meals. In patients with renal impairment, 500mg or 750mg once daily.

Precautions

This medication can rarely cause a serious condition called lactic acidosis; thus, if such symptoms occur, medical personnel should be notified immediately. Caution is also required when using it along with insulin or sulfonylureas, as low blood sugar may occur. This medication should not be used in patients with moderate to severe renal impairment or acute illnesses that may affect renal function, such as dehydration or serious infections. The use of this medication is prohibited in conditions such as acute heart failure, acute myocardial infarction, and sepsis. It is essential to temporarily discontinue this medication before and after undergoing tests that use iodine-based contrast agents and to resume only after normal kidney function is confirmed. This medication is not administered in cases of hypersensitivity or in patients with type 1 diabetes or diabetic ketoacidosis. Care should be taken when used in patients with infections, pre- or post-surgery, malnutrition, liver dysfunction, or excessive alcohol consumption, and it may potentially be temporarily discontinued until the condition improves. Caution is also needed in cases of irregular or insufficient food intake, intense exercise, or other medications that may interact with this medication. Gastrointestinal disturbances are common during treatment with this medication, and if side effects are severe, a doctor should be consulted. Regular monitoring of kidney function, blood sugar levels, and vitamin B12 levels is necessary, especially in elderly patients or those with reduced kidney function. During pregnancy, fetal growth should be closely monitored, and breastfeeding mothers should consider discontinuing nursing or re-evaluating medication treatment. In case of overdose, visit a medical facility immediately for appropriate treatment, as this medication can be removed by blood dialysis. This medication is not recommended for children, and in elderly patients, dosage should be adjusted according to kidney function. Furthermore, alcohol consumption can increase the risk of lactic acidosis, so patients should not indulge in excessive drinking during treatment. This medication may interact with other diabetes treatments and multiple drugs, so if other medications are being taken, it is essential to inform healthcare personnel. The tablet should be swallowed whole for easier intake, and the tablet shell may be excreted, which is normal. During the course of this medication, regular blood sugar tests and kidney function checks should be conducted to continuously monitor treatment effectiveness and side effects.
