Treatment of symptoms of mild to moderate dementia associated with Alzheimer’s disease or Parkinson’s disease
This medication is used to treat mild to moderate symptoms of Alzheimer's-type dementia and dementia symptoms related to Parkinson's disease.
Administration: Rivastigmine is administered orally twice daily with the morning and evening meals. ○ Adults: 1. Treatment of symptoms of mild to moderate Alzheimer-type dementia The initial dose is 3mg of rivastigmine daily. For each patient, increase the dose at intervals of at least 2 weeks up to the highest tolerated dose or the maximum recommended dose of 12mg daily. Since clinical trials showed greater efficacy with daily doses of 6mg and higher, a target dose range of 6~12mg daily is recommended for most patients. Recommended dose titration Administer 1.5mg of this medicine twice daily for at least 2 weeks. If well tolerated, the dose may be increased to 3mg twice daily. Subsequent increases may be made to 4.5mg twice daily and, if well tolerated for at least 2 weeks, to 6mg twice daily. If adverse reactions (e.g., nausea, vomiting, abdominal pain, or loss of appetite) or weight loss occur, one or more doses may be omitted. However, if adverse reactions or weight loss persist, reduce the daily dose to the previous dose that was well tolerated. 2. Treatment of symptoms of mild to moderate dementia associated with Parkinson's disease In controlled clinical trials in patients with dementia associated with Parkinson's disease, 3~12mg of rivastigmine daily was administered in 2 divided doses (1.5~6mg twice daily). The initial dose is 1.5mg of this medicine twice daily. At intervals of at least 4 weeks between doses, subsequent increases may be made according to the patient's tolerability: first to 3mg twice daily, then to 4.5mg twice daily, and then to 6mg twice daily. If adverse reactions (e.g., nausea, vomiting, abdominal pain, or loss of appetite), weight gain, or extrapyramidal symptoms (e.g., tremor) occur during treatment, interrupting treatment for one or more doses may reduce these symptoms. If adverse reactions persist, the daily dose may be temporarily reduced to the previous well-tolerated dose, or discontinuation of treatment may be considered. 3. Maximum recommended dose Administer 6 mg of this medicine orally twice daily. 4. Restarting treatment The incidence and severity of adverse reactions generally increase at higher doses. If treatment is interrupted for more than 3 days, restart at the lowest daily dose and titrate as described above. ○ Patients with hepatic or renal impairment: 1. Exposure is increased in patients with moderate renal impairment or mild to moderate hepatic impairment, and more adverse reactions may occur. Therefore, exercise particular caution when titrating the dose according to individual tolerability. 2. This medicine should not be administered to patients with severe hepatic impairment, as no studies have been conducted in this population.
○ Children: This medicine has not been studied in children; therefore, its use in children is not recommended.
This medication should not be used in patients with allergies to certain medications or components. It should be avoided in patients with severely impaired liver function, as its safety has not been established. Patients with heart disease, peptic ulcers, asthma, urinary retention, seizures, or extrapyramidal disorders should be cautious when using this medication. This medication can commonly cause gastrointestinal side effects such as nausea, vomiting, and diarrhea, especially more frequently at the beginning of treatment. If these symptoms are severe, consult a doctor. Neurological side effects such as dizziness, drowsiness, and headache may also occur, so caution is needed while driving or operating machinery. This medication may affect heart rate, so patients with heart conditions need regular check-ups. Weight loss may occur in dementia patients, so weight changes should be monitored during treatment. Using this medication with other cholinergic drugs may enhance effects or increase side effects, so caution is required during combined therapy. When anesthesia is performed, this medication may enhance the effects of muscle relaxants, so it should be discontinued at appropriate intervals. Women who are pregnant or breastfeeding should consult their doctor about the use of this medication, and especially during breastfeeding, it is preferable to avoid administration. The safety and effectiveness of this medication in children have not been established, thus it is not recommended. In case of overdose, symptoms such as nausea, vomiting, dizziness, bradycardia, confusion may occur, and severe cases require immediate medical attention. This medication should be kept out of the reach of children, and it is safe not to transfer it to other containers.
