Treatment of depression; short-term adjunctive treatment of nicotine dependence during smoking cessation
This medication is used as an adjunct therapy for individuals who wish to quit smoking, helping them over a short period with nicotine dependence. It also assists in treating depression by stabilizing mood and improving symptoms. Thus, it is effectively used for smoking cessation and depression management.
This medicine should be swallowed whole; do not crush, divide, or chew it. Doing so may increase the risk of adverse reactions, including seizures. Short-term adjunctive treatment of nicotine dependence when stopping smoking As an initial dose, administer 150mg once daily for 6 days, then increase to 150mg twice daily. The interval between doses should be at least 8 hours. The maximum dose per administration should not exceed 150mg, and the total daily dose should not exceed 300mg. This medicine should be administered to the patient for at least 7 weeks. If the patient's condition has not progressed significantly toward smoking cessation by week 7 of treatment, consider discontinuing this medicine, as the patient is unlikely to stop smoking during that quit attempt. Combination therapy with this medicine and a nicotine patch: When this medicine is administered in combination with a nicotine patch to treat nicotine dependence, no change in the dosage regimen is necessary. Start combination treatment with the nicotine patch on the target quit date. Before combination treatment, the physician should be thoroughly familiar with the prescribing information for both this medicine and the nicotine patch. Depression It is also important to increase the dose gradually to minimize agitation, extremely restless behavior, and insomnia, which are frequently observed at the beginning of treatment. If necessary, these effects may be managed by temporarily reducing the dose or administering a sedative-hypnotic with an intermediate or long duration of action for a short period; however, a sedative-hypnotic is usually not needed after the first week of treatment. Dosing at times other than bedtime may minimize insomnia, and dose increases should be stopped if the patient complains of distress due to persistent adverse reactions. Initial dose: The usual target dose of this medicine in adults is 150mg twice daily (300mg/day). Administer 1 tablet once in the morning (150mg/day, 150mg once daily) for at least 4 days. If the patient tolerates it adequately, increase the dose to 1 tablet twice daily (300mg/day, 150mg twice daily), allowing an interval of at least 8 hours between doses. Dose increase above 300mg/day: In patients whose symptoms have not improved after treatment with a dose of 300mg/day for several weeks, an increase to a maximum of 400mg/day (200mg twice daily) may be considered. Maintenance dose: Long-term efficacy was demonstrated when patients with recurrent major depressive disorder who had responded to 8 weeks of acute treatment with this medicine were randomized to receive placebo or the same dose of this medicine (150mg twice daily) as maintenance therapy for 44 weeks. Patients with hepatic impairment: Extreme caution should be exercised when administering this medicine to patients with severe cirrhosis, and the dose in these patients should not exceed 100mg daily or 150mg every 2 days. This medicine should be administered with caution to patients with hepatic impairment (including mild to moderate cirrhosis); in patients with mild to moderate cirrhosis, reducing the frequency of administration and/or the dose should be considered. Patients with renal impairment: This medicine should be administered with caution to patients with renal impairment; reducing the frequency of administration and/or the dose should be considered.
