Treatment for improvement of hypercholesterolemia and dyslipidemia (primary, familial) and reduction of cardiovascular disease risk
This medication is primarily used as an adjunct to diet in patients with hypercholesterolemia. It is helpful when primary hypercholesterolemia or mixed hyperlipidemia is not controlled by diet and exercise alone. It is particularly used in patients with homozygous familial hypercholesterolemia, in conjunction with dietary measures or other lipid-lowering therapies. It effectively lowers total cholesterol and LDL cholesterol levels, delaying the progression of atherosclerosis. This medication is also used in pediatric patients aged 10 to 17 with familial hypercholesterolemia who meet certain criteria. It is utilized as an adjunct to dietary measures in patients with primary dysbetalipoproteinemia and helps reduce the risk of stroke, myocardial infarction, and arterial revascularization in high-risk adult patients with elevated high-sensitivity C-reactive protein levels. Therefore, this drug is effectively used for various dyslipidemias and the reduction of cardiovascular disease risk.
For adults, the initial dose is 5 milligrams taken once daily without regard to meals. If necessary, the maintenance dose can be adjusted up to 10 milligrams once daily, and may be increased to a maximum of 20 milligrams once daily based on treatment goals and patient response, with a recommended interval of 4 weeks or more between adjustments.
For pediatric patients aged 10 to 17 years, the dosage is between 5 milligrams and 20 milligrams, administered once daily. The dosage should be adjusted according to the treatment goals of individual patients, with intervals of 4 weeks or more between adjustments. For children under 10 years of age, safety and efficacy have not been established.
This medication should not be used in certain patients. First, patients with an allergy to this medication should not take it. Additionally, individuals with severely impaired liver function or unexplained elevation of liver enzymes should not be given this medication. Patients with muscle disease, those taking the immunosuppressant cyclosporine, and patients with severely reduced kidney function should also avoid this medication. Women who are pregnant or breastfeeding, and women of childbearing age who are not using appropriate contraception should not take this medication. High-dose administration is prohibited in patients at high risk for muscle disease. Patients with genetic metabolic disorders such as lactose intolerance should also avoid this medication. This medication should be used cautiously in patients with a history of excessive alcohol consumption or liver disease. It is important to repeat liver function tests before and during treatment to monitor liver status. Patients with hypercholesterolemia due to hypothyroidism or nephrotic syndrome should use this medication after treating the underlying conditions. This medication can cause myalgia, myopathy, and in rare cases, a serious muscle-wasting condition called rhabdomyolysis. If symptoms such as muscle pain, weakness, or cramps occur during treatment, notify healthcare professionals immediately, and if necessary, blood tests should be performed to check for muscle damage. If there are severe muscle-related adverse effects or significant increases in blood test values, administration of the medication may need to be discontinued. This medication should be administered with caution in patients with reduced kidney function, hypothyroidism, a history of muscle diseases, alcohol abuse, and those aged 70 and older due to the increased risk of adverse muscle reactions. Additionally, this medication may increase blood levels, raising the risk of muscle toxicity when used in conjunction with certain drugs, so it is important to consult with healthcare providers regarding any co-administered medications. In particular, it should be avoided in combination with fibrate drugs, and high-dose administration is prohibited. Safety during pregnancy and breastfeeding has not been established, so it should not be used in these populations, and women of childbearing potential should ensure they are using appropriate contraception. In cases of overdose, there is no specific antidote; thus, treatment should be symptomatic, and liver and muscle enzyme tests should be monitored. This medication does not significantly affect the ability to drive or operate machinery, but caution should be used as dizziness and other symptoms may occur. Finally, during the course of taking this medication, abnormalities such as liver function abnormalities, muscle pain, and skin rashes may occur, and immediate consultation with healthcare professionals is necessary if these symptoms arise.
