Reduction of platelet count and thrombotic risk in myeloproliferative disorders; improvement of related symptoms
This medication is used to improve the symptoms of patients with thrombocythemia due to myeloproliferative disorders. Specifically, it helps to reduce platelet levels, thereby decreasing the risk of thrombus formation within blood vessels and alleviating related symptoms such as thrombosis and bleeding tendencies. Through these effects, it improves the patient's blood circulation status and prevents the occurrence of complications.
For adults, initiate treatment with 0.5mg 4 times daily or 1.0mg twice daily under close monitoring, and maintain this dose for at least 1 week. For both adults and children, administer the lowest effective dose needed to reduce the platelet count and maintain it at 600,000/μL or less, while keeping the platelet count within the normal range as far as possible. If the dose is increased, the increase should be no more than 0.5mg/day, and the dose thus increased should not be increased again within 1 week. The maintenance dose is the same for adult and pediatric patients. Do not exceed 2.5mg as a single dose or 10mg as the total daily dose. To monitor the effect of this medicine and prevent the development of thrombocytopenia, monitor the platelet count every 2 days during the first week of treatment and at least once weekly after reaching the maintenance dose. In general, the platelet count begins to decrease within 7~14 days at the appropriate dose. It takes 4~12 weeks for the platelet count to reach 600,000/μL or less, which is considered a complete response. Most patients will show an adequate response with a daily dose of 1.5~3.0mg. Monitor patients carefully if they have a history of heart disease, renal failure, or hepatic impairment, or if such conditions are suspected.
The initial dose for children ranges from 0.5mg/day to 0.5mg 4 times daily; based on limited data, an initial dose of 0.5mg/day is recommended for children.
This medicine should not be used in patients with severe liver impairment. It may significantly increase the internal concentration of this medicine in patients with moderate to severe liver dysfunction, thus caution is needed. Additionally, patients with genetic disorders that inhibit lactose digestion should not take this medicine as it contains lactose. This medicine should be used with caution in patients with or suspected cardiac disease. It can increase cardiac contractility and heart rate, which may place a burden on the heart. Cardiac function should be assessed before and during treatment, and the cardiac condition should be closely monitored. Patients with impaired liver function should thoroughly evaluate the benefits and risks of this medicine before administration, and the dosage should be adjusted if necessary. Adverse effects related to interstitial lung disease or pulmonary hypertension have been reported; if symptoms such as shortness of breath occur, immediate consultation with healthcare personnel is required. Common side effects include headache, palpitations, nausea, diarrhea, dizziness, depression, forgetfulness, confusion, insomnia, paresthesia, sensory impairment, neurosis, dry mouth, migraine, drowsiness, stuttering, abnormal coordination, diplopia, visual disturbances, tinnitus, increased blood pressure, nosebleeds, gastrointestinal bleeding, gastritis, rash, alopecia, pruritus, skin discoloration, dry skin, joint pain, muscle pain, low back pain, erectile dysfunction, nocturia, fatigue, chest pain, fever, chills, malaise, weakness, and toxicity. Platelet levels can fluctuate sharply, so it is important to frequently monitor platelet counts when discontinuing this medicine. Sudden discontinuation can lead to serious complications such as thrombosis. During treatment with this medicine, regular blood tests, liver function tests, kidney function tests, and electrolyte tests should be conducted. This medicine can lower blood pressure, leading to dizziness or light-headedness. Patients at high risk of bleeding should be especially cautious when taking it with aspirin, and the risks and benefits of concomitant use should be carefully evaluated. This medicine may interact with multiple drugs, so if other medications are being taken, it is essential to inform healthcare personnel. Pregnant women or those who may become pregnant should not take this medicine, and effective contraception should be used during treatment. Breastfeeding women are advised to stop nursing or discontinue the medication as it may be excreted in breast milk. This medication can also be used in pediatric and elderly patients, but caution is advised in the elderly where the incidence of cardiac-related side effects may be higher. In the event of overdose, decreased platelet count and cardiovascular and central nervous system side effects may occur, requiring immediate supervision by healthcare personnel. This medicine should be kept out of reach of children and should not be transferred to another container for safety.
