Treatment of herpes and chickenpox; suppression of recurrence and transmission of genital herpes; prevention of cytomegalovirus infection
This medication is used for the treatment of herpes zoster and herpes simplex infections (initial and recurrent infections of genital herpes). It also helps to suppress the recurrence of genital herpes, aids in safe sexual practices, and reduces transmission. It is effective in preventing cytomegalovirus infections after kidney transplantation and is also used to treat varicella (chickenpox) in children aged from 2 to under 18 with normal immune function. Thus, it is effectively applicable to various viral infection diseases.
1. Adults 1) Herpes zoster: Administer 1,000 mg of valacyclovir per dose orally 3 times daily for 7 days. 2) Genital herpes infection: Administer 500 mg per dose of this drug twice daily. For recurrence, administer for 5 days; for an initial episode, administer for up to 10 days if symptoms are severe. 3) Suppression of recurrent genital herpes infection (1) Patients with normal immune function: The total daily dose is 500 mg of this drug; 250 mg per dose may be administered twice daily. Administration of 500mg per dose once daily is also effective, particularly in patients with fewer than 10 recurrences per year. Some patients who have 10 or more recurrences per year without treatment may obtain additional benefit from divided doses of 250 mg of this drug twice daily (total daily dose 500 mg). Reassess treatment after 6 to 12 months of administration. (2) Immunocompromised patients: Administer 500 mg per dose of this drug twice daily. 4) Reducing transmission of genital herpes: In immunocompetent heterosexual adults with 9 or fewer recurrences per year, the infected sexual partner should take 500 mg per dose of this drug once daily to reduce transmission to a sexual partner who is negative for Herpes Simplex Virus-2 (HSV-2) antibodies. 5) Prevention of cytomegalovirus infection: Take 2,000 mg per dose of this drug 4 times daily, starting within 72 hours after transplantation or as soon as oral administration is possible. The dose should be reduced according to creatinine clearance. Treatment usually lasts 90 days but may need to be extended in high-risk patients. 6) Cold sores: Administer 2,000 mg per dose of this drug twice daily, 12 hours apart, for 1 day. 3. Elderly patients Dose adjustment is not required unless renal function is specifically impaired. Adequate hydration should be maintained. 4. Patients with renal impairment For patients with significantly impaired renal function, adjust the dose as follows. Adequate hydration should be maintained. For patients on hemodialysis, administer after hemodialysis is completed. Herpes zoster: Creatinine clearance 50 or more: 1,000 mg per dose 3 times daily; 30 or more ~ less than 50: 1,000 mg per dose twice daily; 10 or more ~ less than 30: 1,000 mg per dose once daily; less than 10: 500 mg per dose once daily Genital herpes: 30 or more: 500 mg per dose twice daily; less than 30: 500 mg per dose once daily Genital herpes suppression—normal immune function: 30 or more: 500 mg per dose once daily*; less than 30: 250 mg per dose once daily Genital herpes suppression—immunocompromised: 30 or more: 500 mg per dose twice daily; less than 30: 500 mg per dose once daily Cytomegalovirus prevention: 75 or more: 2,000 mg per dose 4 times daily; 50 or more and less than 75: 1,500 mg per dose 4 times daily; 25 or more and less than 50: 1,500 mg per dose 3 times daily; 10 or more and less than 25: 1,500 mg per dose twice daily; less than 10 or on dialysis: 1,500 mg per dose once daily Cold sores: 50 or more: 2,000 mg per dose twice daily; 30 or more and 49 or less: 1,000 mg per dose twice daily; 10 or more and less than 30: 500 mg per dose twice daily; less than 10: 500 mg per dose once daily *Some patients who have 10 or more recurrences per year without treatment may obtain additional benefit from divided doses of 250 mg of this drug twice daily (total daily dose 500 mg). 6. Patients with hepatic impairment No dose adjustment is required in patients with mild to moderate cirrhosis (with normal hepatic synthetic function). No dose adjustment is required in patients with severe cirrhosis (hepatic synthetic dysfunction, with signs of portosystemic shunting), but clinical experience is limited.
Prevention of cytomegalovirus infection in patients aged 12 years and older: Take 2,000 mg per dose of this drug 4 times daily, starting within 72 hours after transplantation or as soon as oral administration is possible. The dose should be reduced according to creatinine clearance. Treatment usually lasts 90 days but may need to be extended in high-risk patients. Cold sores in patients aged 12 years and older: Administer 2,000 mg per dose of this drug twice daily, 12 hours apart, for 1 day. Chickenpox in children aged 2 years to younger than 18 years: Administer 20 mg/kg of this drug per dose 3 times daily for 5 days; the total dose must not exceed 1,000 mg per dose 3 times daily. There are no data on administration of this drug in pediatric patients with creatinine clearance below 50 mL/min/1.73m2.
This drug may cause very serious hematological side effects when administered at high doses to patients with progressive HIV or those undergoing bone marrow and kidney transplants, requiring caution. Patients with hypersensitivity to this medication, its active metabolites, or components should not take it. Particular caution is necessary for patients who are dehydrated, have impaired renal function, hepatic dysfunction, are pregnant or breastfeeding, and the elderly, as dosage adjustments may be required. This medication may cause gastrointestinal symptoms such as nausea, vomiting, diarrhea, and rarely thrombocytopenia, skin rash, itching, and respiratory distress as allergic reactions. Patients with impaired renal function may experience renal impairment or acute renal failure, so adequate hydration is essential, and dialysis may be needed in cases of renal function deterioration. It may affect the central nervous system, causing symptoms such as headache, dizziness, confusion, hallucinations, and seizures; if these symptoms occur, immediate consultation with a physician is required. As safety data for use during pregnancy is limited, it should only be administered when the therapeutic benefit outweighs the risk; it should also be used cautiously in breastfeeding. Elderly patients are at higher risk of side effects due to declining renal function, necessitating dosage adjustment and monitoring of their condition. In the event of overdose, renal failure and neurological symptoms may occur, so it is important to avoid overdose and inform healthcare professionals immediately if unusual symptoms arise. During the course of this medication, it is important to maintain adequate hydration and consult healthcare professionals immediately if any adverse reactions are suspected. Care should be taken when engaging in activities requiring judgment and cognitive abilities, such as driving or operating machinery, as the side effects of this medication may impact these functions.
