Summary

Main use
Treatment of herpes zoster and genital herpes; suppression of recurrent genital herpes
Who takes it
Adults
Adult dose
1. Treatment of herpes zoster Adults: The usual dose of famciclovir is 250 mg per dose, taken orally 3 times daily for 7 days. It is preferable to start treatment as soon as symptoms appear after infection. 2. Treatment of an initial episode of genital herpes Adults: The usual dose of this medicine is 250 mg per dose, taken orally 3 times daily for 5 days. It is preferable to start treatment as soon as symptoms appear after infection. 3. Treatment of an acute recurrent episode of genital herpes Adults: The usual dose of this medicine is 125 mg per dose, taken orally 2 times daily for 5 days. It is preferable to start treatment during the prodromal period or as soon as symptoms appear after infection. 4. Suppression of recurrent genital herpes Adults: The usual dose of this medicine is 250 mg per dose, taken orally 2 times daily. Discontinue treatment periodically every 6~12 months to assess whether the natural course of the disease has changed. 5. Patients with renal impairment: Because creatinine clearance of this medicine is reduced in patients with renal impairment, dosage adjustment should be made with caution. The dosage for these patients is as follows. 1) Treatment of herpes zoster and an initial episode of genital herpes: Creatinine clearance (mL/min/1.73㎡) 30~59 250 mg per dose, 2 times daily 10~29 250 mg per dose, once daily 2) Treatment of an acute recurrent episode of genital herpes: Creatinine clearance (mL/min/1.73㎡) 30~59 125 mg per dose, 2 times daily 10~29 125 mg per dose, once daily 3) Suppression of recurrent genital herpes: Creatinine clearance (mL/min/1.73㎡) 30~59 250 mg per dose, 2 times daily 10~29 125 mg per dose, 2 times daily 4) If only serum creatinine is known, creatinine clearance may be calculated using the following formula. Men: {body weight (kg) × (140-age)}/ {72 × serum creatinine (mg/dL)} Women: 0.85 × the value calculated for men 5) Patients with renal impairment undergoing hemodialysis For patients undergoing hemodialysis, administration every 48 hours is preferable, taking the dialysis interval into account. Since 4 hours of hemodialysis reduces the plasma concentration of this medicine by approximately 75%, administer this medicine immediately after dialysis. The recommended dose is 250mg for patients with herpes zoster and 125mg for patients with genital herpes. 6. Patients with hepatic impairment: No dosage adjustment is required for patients with chronic compensated liver disease. There is insufficient treatment experience in patients with evident chronic decompensated liver disease; therefore, no clear recommended dose has been established.

Precautions

This medication should not be administered to certain patients. Patients allergic to this medication or its components should not use it. Additionally, this medication contains lactose, so patients with genetic issues such as galactose intolerance, Lapp lactase deficiency, and glucose-galactose malabsorption should not take it. Patients with impaired kidney function should use this medication with caution. Taking this medication may result in various side effects, including headaches, nausea, vomiting, dizziness, skin rash, and liver dysfunction. Especially in older adults, symptoms such as drowsiness or confusion may occur more frequently, so caution is required. Serious side effects such as thrombocytopenia or acute renal failure have also been reported. This medication does not cure genital herpes, so it is recommended to avoid sexual contact and maintain safe sexual practices during treatment. Furthermore, if dizziness or drowsiness occurs while taking this medication, driving or operating machinery should be avoided. Patients with mild to moderate liver dysfunction do not need special dosage adjustments, but those with severe liver dysfunction should be cautious as the efficacy may decrease. The likelihood of interactions with other medications is low, but when combined with drugs such as probenecid, blood levels may increase, necessitating monitoring for toxicity. Also, when used with lamotrigine, the therapeutic effect should be closely observed. Women who are pregnant or may become pregnant should only use this medication when the benefits outweigh the risks, and nursing mothers should also be administered carefully. Children under 18 years old should generally avoid this medication as its safety and efficacy have not been established. For older adults, no dosage adjustment is generally necessary, but dosage adjustments should be made if kidney function is impaired. In case of overdose, appropriate supportive and symptomatic treatment is needed, as hemodialysis can significantly reduce the concentration of the active ingredient in this medication. This medication should be stored in a tightly closed container to protect it from air and moisture. It should also be placed in a dry location at or below 30 degrees Celsius, and transferring to other containers is not recommended as it may adversely affect quality.
