Summary

Main use
Treatment of genital herpes and suppression of recurrence; treatment of herpes zoster
Who takes it
Adults
Adult dose
1. Treatment of herpes zoster Adults: Administer 250 mg of famciclovir orally 3 times daily for 7 days. It is preferable to start treatment as soon as symptoms appear after infection. 2. Treatment of initial genital herpes infection Adults: Administer 250 mg of this medicine orally 3 times daily for 5 days. It is preferable to start treatment as soon as symptoms appear after infection. 3. Treatment of acute recurrent genital herpes infection Adults: Administer 125 mg of this medicine orally 2 times daily for 5 days. It is preferable to start treatment during the prodromal period or as soon as symptoms appear after infection. 4. Suppression of recurrent genital herpes Adults: Administer 250 mg of this medicine orally 2 times daily. Interrupt treatment periodically every 6~12 months to assess whether the natural course of the disease has changed. 5. Patients with renal impairment: Because the clearance of this medicine is reduced in patients with renal impairment, dose adjustment should be made with caution. The dosage for these patients is as follows. 1) Treatment of herpes zoster and initial genital herpes infection: Creatinine clearance (mL/min/1.73㎡) Dose 30~59 250 mg per dose, 2 times daily 10~29 250 mg per dose, once daily 2) Treatment of acute recurrent genital herpes infection: Creatinine clearance (mL/min/1.73㎡) Dose 30~59 125 mg per dose, 2 times daily 10~29 125 mg per dose, once daily 3) Suppression of recurrent genital herpes infection: Creatinine clearance (mL/min/1.73㎡) Dose 30~59 250 mg per dose, 2 times daily 10~29 125 mg per dose, 2 times daily 4) If only serum creatinine is known, creatinine clearance can be calculated using the following equations. Men: {body weight (kg) × (140-age)}/ {72 × serum creatinine (mg/dL)} Women: 0.85 × the value calculated for men 5) Patients with renal impairment undergoing hemodialysis For patients on hemodialysis, administration every 48 hours is preferable, taking the dialysis interval into account. Since 4 hours of hemodialysis reduces the plasma concentration of this medicine by approximately 75%, administer this medicine immediately after dialysis. The recommended dose is 250mg for patients with herpes zoster and 125mg for patients with genital herpes. 6. Patients with hepatic impairment: No dose adjustment is required for patients with chronic compensated liver disease. There is insufficient treatment experience in patients with clear chronic decompensated liver disease; therefore, no specific recommended dose has been established.

Precautions

This medication should not be used in patients who have had allergic reactions to this drug or to its active ingredient, penciclovir. Patients with impaired kidney function need to be cautious when using this drug, and dosage adjustments may be necessary. Headaches or nausea may occur while taking this medication, but they are mostly mild or moderate and not severe. Regular blood tests may be necessary as platelet or white blood cell counts may decrease. Neurological symptoms such as dizziness, drowsiness, and confusion may occur; it's advisable to avoid driving or operating machines if these symptoms arise. Gastrointestinal symptoms such as nausea, vomiting, abdominal pain, and diarrhea can commonly occur. Liver function tests may show abnormalities or, rarely, symptoms of jaundice may develop, so the liver's health status should be monitored closely. Skin rash or itching may occur frequently, and severe skin reactions may rarely occur, so skin conditions should be carefully monitored. Acute kidney failure has been reported rarely in patients with impaired renal function, thus they should check their kidney health while taking this medication. This medication does not cure genital herpes completely, so it is important to avoid sexual contact when symptoms are present and to maintain safe sexual practices. The likelihood of affecting driving or machine operation is low, but caution is advised if dizziness or drowsiness occurs. Patients with mild to moderate liver function impairment do not generally require specific dosage adjustments; however, patients with severe liver impairment may experience reduced efficacy. Drug interactions are relatively few; however, caution should be taken regarding changes in blood concentration when used concurrently with drugs eliminated by the kidneys. During pregnancy, safety has not been established, so it should be used only when necessary, and breastfeeding women should also consult a doctor before taking it. It is not recommended for use in children under 18 years, as safety and effectiveness have not been established. For the elderly, dosage adjustments are usually not necessary, but adjustments may be needed in cases of impaired renal function. In cases of overdose, appropriate treatment is needed and the drug concentration can be reduced by hemodialysis. Store this medication in a closed container as it is sensitive to air and moisture, and keep it in a cool, dry place below 30 degrees Celsius, out of reach of children. Avoid transferring to another container as it may affect the quality.
