Treatment for hypercholesterolemia and reduction of cardiovascular disease risk
This drug is primarily used in patients with hypercholesterolemia and hyperlipidemia. It is used as an adjunct therapy to lower blood cholesterol levels when primary hypercholesterolemia or mixed hyperlipidemia cannot be controlled by diet and exercise. It is also beneficial for patients with heterozygous familial hypercholesterolemia or homozygous familial hypercholesterolemia alongside dietary measures. It is used to reduce cholesterol and associated protein levels in children aged 10 to 17 with certain conditions. Additionally, it is effective in reducing the risk of stroke, myocardial infarction, and vascular revascularization in middle-aged men and women at high risk for cardiovascular disease. Thus, this drug is used to reduce hyperlipidemia and the risk of cardiovascular diseases.
This drug can be administered at any time of day, regardless of meals. The initial dose is usually once daily 5 mg, while a maintenance dose of once daily 10 mg is appropriate for most patients. The maintenance dose should be adjusted based on the LDL-cholesterol levels, treatment goals, and patient response, with increments up to a maximum of 20 mg once daily, at intervals of 4 weeks or longer.
The usual dosage range for pediatric patients with heterozygous familial hypercholesterolemia (ages 10 to 17) is once daily 5 mg to 20 mg. The dosing should be adjusted individually according to the recommended treatment goals.
This drug should not be administered to certain patients. For example, it should not be used in patients who are allergic to any component of this drug, patients with severely impaired liver function, patients with muscle disorders, and those with significantly reduced kidney function. Special caution is necessary for pregnant or breastfeeding women and for fertile women who are not using proper contraception. High doses are prohibited in patients at increased risk of muscle disorders, such as those with moderate or severe renal impairment, hypothyroidism, a history of muscle disorders, or alcohol dependence. This drug contains lactose, so it should not be administered to patients with lactose intolerance or related genetic disorders. Regular monitoring of liver health is required when taking this drug, and patients with a history of significant alcohol intake or liver disease should be especially cautious. Patients with hypothyroidism or nephrotic syndrome should treat the underlying disorders before taking this drug. This drug may affect the muscles and can cause serious muscle damage, such as myalgia, myopathy, and, rarely, rhabdomyolysis. If symptoms like myalgia, weakness, or cramps appear, immediately inform a doctor, and blood tests may be necessary to check for muscle damage. Patients at higher risk of muscle damage (e.g., those with renal impairment, the elderly, hypothyroidism, and a history of muscle disorders) should be monitored closely before and after treatment. Some patients may experience transient proteinuria, but this usually does not lead to serious kidney damage. However, regular kidney function tests are recommended with high-dose administration. Asian patients may have increased systemic exposure to this drug, necessitating dose adjustments. While taking this drug, there may be rare side effects such as headache, dizziness, constipation, nausea, abdominal pain, rash, itching, myalgia, generalized weakness, diarrhea, fatigue, numbness, paresthesia, chest discomfort, anorexia, abdominal distension, fainting, generalized pain, muscle cramps, gout, erectile dysfunction, hematuria, thrombocytopenia, amnesia, peripheral neuropathy, cough, sexual dysfunction, gynecomastia, depression, sleep disorders (including insomnia and nightmares), and cognitive impairment. Serious side effects including liver dysfunction, severe skin reactions, severe myasthenia, interstitial lung disease have been reported, and immediate discontinuation of treatment and medical evaluation are required if these symptoms occur. This drug may interact with other medications, so caution is needed when taking it with cyclosporine, fibrates, certain antibiotics, antifungals, antivirals, etc. This drug should not be used during pregnancy, and women who may become pregnant should use appropriate contraception. It is also deemed safe for breastfeeding women to avoid taking this drug. In case of overdose, there is no specific antidote, so treatment should focus on symptoms, and liver and muscle function tests are necessary. Caution is required when driving or operating machinery as dizziness can occur. Before taking this drug, you must consult your doctor or pharmacist to inform them about your health status and any medications you are currently taking, and it should be used safely through regular check-ups and symptom monitoring.
