Treatment and suppression of recurrence of genital herpes; treatment of herpes zoster
This medication is used to treat infections caused by the herpes zoster virus and genital herpes. It also helps in suppressing the occurrence of recurrent genital herpes.
1. Treatment of herpes zoster Adults: Administer famciclovir 250 mg per dose orally, 3 times daily for 7 days. It is preferable to start treatment as soon as symptoms appear after infection. 2. Treatment of initial genital herpes infection Adults: Administer this medicine 250 mg per dose orally, 3 times daily for 5 days. It is preferable to start treatment as soon as symptoms appear after infection. 3. Treatment of acute recurrent genital herpes infection Adults: Administer this medicine 125 mg per dose orally, 2 times daily for 5 days. It is preferable to start treatment during the prodromal period or as soon as symptoms appear after infection. 4. Suppression of recurrent genital herpes Adults: Administer this medicine 250 mg per dose orally, 2 times daily. Periodically discontinue treatment every 6~12 months to assess whether the natural course of the disease has changed. 5. Patients with renal impairment: Because the clearance of this medicine decreases in patients with renal impairment, use caution when adjusting the dose. The dosage and administration for these patients are as follows. 1) Treatment of herpes zoster and initial genital herpes infection: Creatinine clearance (mL/min/1.73㎡) Dose 30~59 250 mg per dose, 2 times daily 10~29 250 mg per dose, once daily 2) Treatment of acute recurrent genital herpes infection: Creatinine clearance (mL/min/1.73㎡) Dose 30~59 125 mg per dose, 2 times daily 10~29 125 mg per dose, once daily 3) Suppression of recurrent genital herpes infection: Creatinine clearance (mL/min/1.73㎡) Dose 30~59 250 mg per dose, 2 times daily 10~29 125 mg per dose, 2 times daily 4) If only serum creatinine is known, creatinine clearance can be calculated using the following formula. Men: {body weight (kg) × (140-age)}/ {72 × serum creatinine (mg/dL)} Women: 0.85 × the value calculated for men 5) Patients with renal impairment undergoing hemodialysis For patients undergoing hemodialysis, it is preferable to administer the medicine every 48 hours, taking the dialysis interval into account. A 4-hour hemodialysis session reduces the plasma concentration of this medicine by approximately 75%; therefore, administer this medicine immediately after dialysis. The recommended dose is 250mg for patients with herpes zoster and 125mg for patients with genital herpes. 6. Patients with hepatic impairment: No dose adjustment is required in patients with chronic compensated liver disease. There is insufficient treatment experience in patients with overt chronic decompensated liver disease; therefore, no clear recommended dose has been established.
This medication should not be administered to certain patients. Individuals who have had an allergic reaction to this drug or its components, particularly penciclovir, should not take it. Furthermore, patients with a genetic disorder that prevents lactase from breaking down lactose should avoid this medication. Serious side effects, though rare, include thrombocytopenia, leukopenia, dizziness, drowsiness, hallucinations, palpitations, liver function abnormalities, jaundice, and acute renal failure. This medication does not cure genital herpes, so it is important to avoid sexual contact and maintain safe sex practices if symptoms occur. Patients with severely impaired liver function may experience reduced efficacy, so caution is warranted. Patients with high blood lipid levels should undergo regular tests and adjust the dose if necessary. Women who are pregnant or could become pregnant should consult a doctor before taking this medication, and breastfeeding mothers should also be cautious while taking it. Generally, children under 18 should not take this drug as its safety and efficacy have not been established. Elderly patients typically do not require dose adjustments, but those with reduced kidney function should have their doses adjusted. In case of overdose, one should notify healthcare professionals immediately and receive appropriate treatment; blood dialysis can help lower the drug concentration. This medication should be stored in a tightly sealed container to avoid exposure to air and moisture, and it is important to keep it in a cool, dry place below 30 degrees Celsius, away from children's reach and avoid transferring it to other containers.
