Treatment of hypercholesterolemia (primary, familial) and reduction of risk for cardiovascular diseases
This medication is used as an adjunct when diet and exercise are not sufficient to control primary hypercholesterolemia and mixed dyslipidemia. It is also helpful as an adjunct to diet or other lipid-lowering therapies in patients with familial hypercholesterolemia. In particular, it lowers total cholesterol and LDL-cholesterol levels in patients with hypercholesterolemia, delaying the progression of atherosclerosis. It is effective in pediatric patients with heterozygous familial hypercholesterolemia aged 10 to 17 years and is used to reduce total cholesterol, LDL-cholesterol, and Apo-B protein levels. It is also suitable as an adjunct to diet for patients with primary dysbetalipoproteinemia. Additionally, it helps reduce the risk of stroke, myocardial infarction, and vascular revascularization procedures in men over 50 and women over 60 with elevated high-sensitivity C-reactive protein levels. This medication is used to reduce the risk of cardiovascular diseases related to dyslipidemia and improve health.
For patients with primary hypercholesterolemia, mixed dyslipidemia, and dysbetalipoproteinemia, the initial dose is 5 mg once daily, at any time of the day regardless of meals. If more LDL-cholesterol reduction is needed, the maintenance dose can be adjusted, and most patients are managed at a dose of 10 mg once daily. The maintenance dose should be adjusted based on LDL-cholesterol levels and treatment goals at intervals of at least 4 weeks.
In pediatric patients with heterozygous familial hypercholesterolemia (ages 10 to 17), the dosage is 5 mg to 20 mg once daily. The dosage should be adjusted according to the individual patient's treatment goals and liver function at intervals of at least 4 weeks.
This drug should not be administered to certain patients. For example, it is contraindicated in individuals with allergies to the components of this drug, those with liver function issues, muscle diseases, and patients with severely impaired kidney function. Women who are pregnant or breastfeeding, as well as women of childbearing potential who are not using appropriate contraception, should not take this medication. It should only be administered to women of childbearing age after explaining the low likelihood of pregnancy and fetal risks. Patients at high risk for muscle diseases, such as those with moderate to severe kidney impairment, hypothyroidism, a family history of muscle diseases, or alcoholics, should avoid high doses of this medication. This medication contains lactose and should not be administered to patients with lactose intolerance or certain genetic metabolic disorders. While taking this drug, patients with a history of excessive alcohol intake or liver disease need regular liver function tests. Patients with hypercholesterolemia due to hypothyroidism or nephrotic syndrome should first treat the underlying disease. Myalgia, myopathy, and rarely severe muscle breakdown (rhabdomyolysis) may occur; therefore, if muscle pain or weakness occurs, the physician must be notified immediately, and if necessary, blood tests should be conducted to check for muscle damage. This drug may interact with other medications; caution is especially advised when used in conjunction with fibrates, certain antibiotics, antifungals, and immunosuppressants. The combination with specific drugs may increase the blood concentration of this drug, thereby increasing the risk of side effects. Pregnant or breastfeeding women should not use this medication, and women who could become pregnant must use appropriate contraception. In case of overdose, there is no specific antidote, so symptomatic treatment and monitoring of liver function and muscle enzymes are necessary. This drug does not significantly affect the ability to drive or operate machinery, but caution should be exercised as dizziness may occur. Finally, this drug may rarely cause serious skin reactions, liver dysfunction, kidney problems, and other severe side effects, so if any unusual symptoms occur, consult healthcare personnel immediately.
Allergic reactions including urticaria and angioedema, severe swelling (edema), tightness in the chest, difficulty breathing, respiratory distress, jaundice, hepatitis, increased transaminases, Stevens-Johnson syndrome, drug reaction with eosinophilia and systemic symptoms (DRESS), rhabdomyolysis, immune-mediated necrotizing myopathy, thrombocytopenia, severe muscle weakness, ocular myasthenia, polyneuropathy, peripheral neuropathy, kidney failure, acute tubular necrosis
