Treatment of hypercholesterolemia (primary, familial) and cardiovascular disease risk reduction
This medicine is used as an adjunct when dietary control and exercise are insufficient to manage patients with primary hypercholesterolemia and mixed dyslipidemia. It is also utilized as an adjunct in patients with homozygous familial hypercholesterolemia. It reduces total cholesterol and LDL-cholesterol levels in patients with hypercholesterolemia, delaying the progression of atherosclerosis, and helps lower blood lipid levels in pediatric patients aged 10 years or older with heterozygous familial hypercholesterolemia. Additionally, it is used as an adjunct in dietary therapy for patients with primary dysbetalipoproteinemia. In adult patients with elevated high-sensitivity C-reactive protein and additional cardiovascular disease risk factors, it has the effect of reducing the risk of stroke, myocardial infarction, and arterial revascularization. It is effectively used in various dyslipidemia and cardiovascular disease risk groups.
For primary hypercholesterolemia and mixed dyslipidemia, it can be taken at any time of the day, regardless of meals, once daily. The initial dosage is 5 mg once daily, and the maintenance dosage is 10 mg once daily, with most patients being adjusted at this dosage. The maintenance dosage should be adjusted according to LDL-cholesterol levels, treatment goals, and patient response at intervals of 4 weeks or more, with a maximum daily dosage up to 20 mg.
For pediatric patients with heterozygous familial hypercholesterolemia (ages 10 to 17), it is administered once daily in a range of 5 mg to 20 mg. The dosage is suggested to be adjusted at intervals of 4 weeks or more.
This medication should not be used in certain patients. For example, patients who are allergic to this medication, those with severely impaired liver function, patients with muscle diseases, patients with severely reduced kidney function, women who are pregnant or breastfeeding should not take this medication. Moreover, high-dose use is prohibited in patients at risk of myopathy. This medication contains lactose, therefore it should not be taken by patients with lactose intolerance. While using this medication, careful observation of liver function tests and muscle-related symptoms is required. If muscle pain or weakness symptoms appear, immediately notify the healthcare provider, and cessation of the medication may be necessary. This medication may interact with other drugs, and caution is needed especially when used in conjunction with certain antiviral drugs, immunosuppressants, antifungal agents, antibiotics, and fibrates. Women of childbearing potential must employ appropriate contraception before taking this medication, and it should not be taken during pregnancy. It should also be avoided while breastfeeding. In case of overdose, there are no specific antidotes. Symptomatic treatment and assessment of liver and muscle function are necessary. This medication may cause dizziness, so it is important to be cautious when driving or operating machinery. Lastly, this medication may show higher blood concentration levels in Asian patients; thus, dose adjustments may be required.
Allergic reactions including urticaria and angioedema, chest tightness, difficulty breathing, severe rash with fever, blisters, skin peeling, pain in oral/mucosal areas, Stevens-Johnson syndrome, drug reaction with eosinophilia and systemic symptoms (DRESS), difficulty breathing or shortness of breath, asthma attacks, respiratory issues accompanied by cough and fever, yellowing of the skin or eyes (jaundice), severe lethargy, sudden
