Take 40mg orally once daily at least 1 hour before a meal or at least 3 hours after a meal. This medicine must be administered without food, and the tablet should be swallowed whole with water. This medicine may be continued until disease progression or unacceptable toxicity occurs. Temporarily interrupt treatment with this medicine in the event of NCI CTCAE grade 3 or higher, grade 2 or higher diarrhea persisting for more than 2 days despite antidiarrheal treatment, grade 2 skin reactions persisting for more than 7 days or that are intolerable, or grade 2 or higher renal impairment. When the adverse drug reaction recovers to grade 1 or lower, resume treatment with this medicine at a dose reduced by 10mg from the previously administered dose. Permanently discontinue treatment with this medicine in the event of severe bullous, blistering, or exfoliative skin symptoms, interstitial lung disease, severe hepatic impairment, persistent ulcerative keratitis, symptoms of left ventricular dysfunction, or severe or intolerable adverse reactions at a daily dose of 20mg. No dose adjustment is necessary in patients with mild (glomerular filtration rate (eGFR) 60-89mL/min) or moderate (glomerular filtration rate (eGFR) 30-59mL/min) renal impairment. For patients with severe renal impairment (glomerular filtration rate (eGFR) 15-29mL/min), a dose of 30mg once daily is recommended. Administration of this medicine is not recommended in patients with a glomerular filtration rate (eGFR) below 15mL/min or in patients undergoing dialysis. No adjustment of the starting dose is necessary in patients with mild hepatic impairment (Child Pugh A) or moderate hepatic impairment (Child Pugh B). As this medicine has not been studied in patients with severe hepatic impairment (Child Pugh C), its administration is not recommended. For patients who must receive a P-glycoprotein (P-gp) inhibitor, it should be administered with this medicine or after administration of this medicine. If the tablet cannot be swallowed whole, it may be dispersed in approximately 100ml of non-carbonated drinking water; do not use any other liquid. Do not crush this medicine. Place it in water and stir occasionally for 15 minutes until the tablet breaks into very small particles, then take the dispersion immediately. Rinse the cup used with approximately 100ml of water and drink it. This dispersion may also be administered through a feeding tube.
The safety and efficacy of this drug have not been studied in children; therefore, its use is not recommended in pediatric patients.
This drug may cause harmful effects to the fetus if taken during pregnancy; therefore, pregnant women should not use it. Patients with a history of interstitial lung disease or severely compromised liver function should use this medication cautiously and discontinue if symptoms worsen and seek treatment. There may be interactions with other medications, so caution is required, especially when combined with P-glycoprotein inhibitors or inducers. Diarrhea, skin rashes, stomatitis, etc., may commonly occur as side effects, and if these symptoms are severe, consultation with a doctor for dose adjustment or treatment may be necessary. Adequate fluid intake should be maintained during treatment; if diarrhea occurs, antidiarrheal agents should be used immediately, and medication may need to be temporarily suspended if necessary. Exposure to sunlight may worsen skin reactions; using sunscreen and wearing protective clothing is advised.
