Treatment of hypercholesterolemia and reduced cardiovascular disease risk
This medication is primarily used to manage dyslipidemia, including hypercholesterolemia and mixed hyperlipidemia. It serves as an adjunct when diet and exercise alone are insufficient, particularly effective in patients with primary hypercholesterolemia and heterozygous familial hypercholesterolemia. Additionally, it helps reduce the incidence of cardiovascular diseases such as strokes and myocardial infarctions in patients with cardiovascular risk factors. It can be used in pediatric patients who meet specific criteria in conjunction with diet. Thus, this medication effectively lowers cholesterol levels to improve vascular health and delay the progression of cardiovascular diseases.
The initial dosage of 5 mg should be taken once daily at any time, irrespective of meals. If a greater reduction in LDL cholesterol is needed, the dosage may be adjusted to 10 mg once daily, with most patients being well-controlled at this dosage. The dosage should be appropriately adjusted every 4 weeks or longer according to LDL cholesterol levels, treatment goals, and patient response, with a maximum daily dose of 20 mg.
Pediatric patients aged 10 to 17 years may be administered 5 mg to 20 mg once daily. The dosage should be adjusted according to the individual patient's treatment goals, with intervals of 4 weeks or longer for adjustments.
This medication should not be used in patients with allergies to specific ingredients. Patients with poor liver function or liver diseases should be cautious as it may worsen liver function. Patients with muscle diseases or problems may face a higher risk of muscle damage while taking this medication. Coadministration with certain drugs such as cyclosporine increases the risk of side effects, and it should be avoided. Patients with severe kidney impairment are contraindicated from using this medication. Pregnant or breastfeeding women, and those planning to become pregnant, should not take this medication, and women of childbearing potential must use appropriate contraception. The 40 mg dosage is contraindicated in patients at high risk of muscle-related issues and should be used cautiously in those with moderate to severe kidney impairment, hypothyroidism, or family history thereof. This medication contains lactose, so it should not be used in patients with genetic issues such as lactose intolerance. Patients with excessive alcohol intake or a history of liver disease require regular liver function tests and should take this medication only after cause-specific treatment for hypothyroidism or nephrotic syndrome. If muscle pain, weakness, or cramps occur, notify a doctor immediately and discontinue use if symptoms are severe. It is safer to discontinue use in acute severe conditions, sepsis, or after major surgeries. This medication can rarely cause severe muscle weakness or ophthalmoplegia; if symptoms occur, it should be discontinued immediately. The risk of muscle disease may increase when used with certain other medications, so caution is required when co-administering. Kidney function tests are needed for high-dose use, and dosages may need to be adjusted for Asian patients due to higher drug concentrations. In pediatric and adolescent cases, there may be influences on growth and development requiring caution. Rare side effects, such as interstitial lung disease, severe liver damage, and severe skin reactions, may occur while taking this medication, so if they arise, it should be immediately discontinued and healthcare professionals consulted. This medication can interact with other drugs, so it is crucial to inform providers of any medications currently being taken, especially those that pose a risk of muscle damage. Pregnant or breastfeeding women should not take this medication, and if pregnancy is planned, appropriate contraception must be used. In the case of overdose, there is no specific antidote, so treatment should be given according to symptoms, and liver function and muscle enzyme tests must be performed to monitor the condition. Symptoms such as dizziness may occur when driving or operating machinery, so caution is advised.
Allergic reactions including urticaria and angioedema, Stevens-Johnson syndrome, drug reaction with eosinophilia and systemic symptoms (DRESS), rhabdomyolysis, severe muscle weakness, ophthalmoplegia, myopathy including myositis, jaundice, hepatitis, severe allergic reactions, severe liver damage, hyperbilirubinemia, acute renal failure, oliguria, thrombocytopenia, severe rash with high fever, blisters, skin peeling, oral/ocular symptoms
