Treatment of severe symptomatic ventricular tachyarrhythmia and symptomatic supraventricular tachyarrhythmia
This medication is used for symptomatic loss of ventricular rhythm, specifically for tachycardia associated with Wolff-Parkinson-White syndrome and paroxysmal atrial fibrillation, as well as for the treatment of severe symptomatic ventricular arrhythmias that are life-threatening or require medical treatment based on a physician's judgment. It helps to improve these cardiac arrhythmias and stabilize heart rhythm.
The individual maintenance dose should be determined under cardiac monitoring, including ECG monitoring and repeated blood pressure measurements. Adults: Administer 450-600㎎ of propafenone hydrochloride orally per day (one 150mg tablet 3 times daily or one 300mg tablet twice daily). In some cases, the dose may be increased up to 900㎎ per day (one 300mg tablet or two 150mg tablets 3 times daily). This dosage regimen applies to patients weighing approximately 70㎏. For patients weighing less, the daily dose should be reduced according to body weight. The daily dose may be exceeded only in exceptional circumstances and under strict supervision by a cardiologist. In particular, in elderly patients, patients with a history of myocardial injury, and patients with hepatic or renal impairment, initiate treatment at a low dose and increase it with caution while monitoring whether blood concentrations are appropriate. The first dose increase should be made after at least 3-4 days. Patients with ventricular arrhythmias need careful cardiac examinations during the initial phase of treatment. In these patients, treatment should be initiated only when emergency cardiac equipment is available and monitoring can be assured, and regular examinations are required during treatment (for example, standard ECG at monthly intervals, Holter monitoring every 3 months, and exercise ECG if possible). If ECG changes occur, such as QRS widening or QT prolongation greater than 25%, PR prolongation greater than 50%, QT prolongation greater than 500ms, or an increase in the frequency or severity of arrhythmia, decide whether to continue treatment. This medicine has a bitter taste and surface anesthetic properties; swallow it after meals with a small amount of liquid. When prescribing this medicine, consider that there is no conclusive evidence that treatment with ClassⅠ antiarrhythmic agents helps prolong life.
This drug is used to treat cardiac arrhythmias and may be hazardous for patients with structural heart problems or those who have recently experienced a myocardial infarction. It should not be administered to patients with severely decreased left ventricular function, cardiogenic shock, Brugada syndrome, etc. Furthermore, patients with severe bradycardia, low cardiac output, or sinoatrial node dysfunction should not use this drug. Caution is also needed for patients with impaired liver or kidney function, elderly patients, or those with obstructive airway disease. During treatment, regular monitoring of ECG and blood pressure is necessary. While taking this medication, cardiac-related side effects such as decreased heart rate, conduction disorders, and ventricular tachycardia may occur; if any unusual symptoms arise, immediate consultation with a physician is required. Other reported side effects include dizziness, nausea, fatigue, liver function abnormalities, etc., so caution is advised. This medication may interact with other heart medications or specific antidepressants and antibiotics; if you are taking other medications, be sure to inform your healthcare provider. During pregnancy, it should be used only when absolutely necessary for the safety of the fetus and mother, and during breastfeeding, administration should be avoided if possible or breastfeeding should be discontinued. The safety for use in children has not been sufficiently verified; thus, caution is needed. In the case of overdose, severe cardiac side effects and central nervous system symptoms can appear, necessitating immediate emergency treatment. Caution is advised when driving or operating machinery while taking this medication, and alcohol consumption may increase the risk of side effects.
