Treatment of hyperprolactinemia; prevention and suppression of milk secretion
This medication is primarily used to prevent or suppress milk secretion after childbirth. For instance, it can help when a mother does not wish to breastfeed or cannot breastfeed for medical reasons, or to suppress milk secretion following stillbirth or miscarriage. It is also used to treat symptoms related to amenorrhea, oligomenorrhea, anovulation, and galactorrhea associated with the condition known as hyperprolactinemia. Hyperprolactinemia refers to a state where the hormone prolactin is excessively secreted and can occur due to pituitary adenomas, idiopathic hyperprolactinemia, or Empty Sella Syndrome. This medication is effective in normalizing prolactin levels and alleviating symptoms in these various situations.
This medicine is taken orally. Dopamine agonists such as this medicine are better tolerated when taken with food; therefore, for all indications, take this medicine immediately after meals. 1. Prevention and suppression of lactation 1) Suppression of lactation: Take a single dose of cabergoline 1mg on the first day postpartum 2) Suppression of established lactation: Take cabergoline 0.25mg every 12 hours for 2 days (total 1mg) 2. Treatment of hyperprolactinemia: The recommended initial dose is cabergoline 0.5mg 1~2 times per week (e.g., 1/2 tablet on Monday and Thursday). Gradually increase the dose by 0.5mg of cabergoline per week, preferably at monthly intervals, until an optimal therapeutic response is achieved. The usual therapeutic dose is cabergoline 1mg per week, administered within a range of 0.5mg ~ 2mg per week. However, the maximum dose should not exceed cabergoline 4.5mg per week or 3mg per day. Adjust the dosing frequency to once or 2 or more times per week according to the patient's adherence. Tolerability when taking more than cabergoline 1mg once weekly has been evaluated in only a small number of patients; therefore, dividing doses into several administrations is recommended when taking more than cabergoline 1mg per week. During dose escalation, evaluate the patient to determine the minimum dose that produces a therapeutic response. Monitor serum prolactin levels monthly. After reaching an effective therapeutic dose, serum prolactin levels usually return to normal within 2~4 weeks. Symptoms of hyperprolactinemia may recur after this medicine is discontinued. In some patients, prolactin levels have been observed to remain low for several months. In a follow-up of a group of women who discontinued this medicine, ovulatory cycles continued for at least 6 months after discontinuation in 23 of 29 women.

Impulse control disorders may occur, so patients and caregivers should closely monitor for changes in behavior. Pregnant women or those planning to become pregnant should avoid taking this medication and should avoid pregnancy for at least one month after discontinuation. As this medication acts on dopamine receptors, co-administration with dopamine antagonists or certain antibiotics may reduce its effectiveness or increase side effects. Therefore, caution should be exercised to avoid taking these medications concurrently. The safety and efficacy for children or adolescents under 19 years have not been confirmed, and information regarding the elderly is also limited.
