Treatment of psychiatric disorders (depression and obsessive-compulsive disorder), premenstrual dysphoric disorder, and bulimia nervosa
This medication is effective in improving the persistent depressive state and associated symptoms in patients with depression. It particularly helps alleviate symptoms such as changes in appetite and sleep, changes in psychomotor activity, reduced interest and libido, fatigue, guilt, cognitive slowing, and suicidal thoughts. It also effectively reduces binge eating and purging behaviors in patients with bulimia nervosa and significantly alleviates symptoms of obsessive-compulsive disorders. It is also used for improving depression, anxiety, emotional instability, and physical symptoms related to premenstrual dysphoric disorder. It is important to regularly assess the treatment effect with a physician during long-term usage. This medication is used to treat depression, bulimia nervosa, obsessive-compulsive disorders, and premenstrual dysphoric disorder.
Adults 1. Depression 1) Initial treatment: The initial dose is fluoxetine hydrochloride 20mg orally once daily in the morning. In controlled clinical trials, doses of 20~80mg per day were administered. If no improvement is observed, an increase in dose may be considered after several weeks. Doses exceeding 20mg per day should be administered in 2 divided doses (morning and noon), and the maximum dose of 80mg per day should not be exceeded. The onset of an adequate antidepressant effect may be delayed until 4 weeks or longer after starting treatment. 2) Maintenance/continuation/extended treatment: There are no definitive data on how long patients should be treated with this medication, but psychopharmacologists recognize that acute symptoms of depression require drug treatment for several months or longer. It is not known whether the amount of antidepressant needed to relieve symptoms is the same as that needed to maintain or sustain remission. 2. Bulimia nervosa Administer 60mg of this medication per day. The daily dose should not exceed 80mg. Measure electrolyte levels before starting treatment. 3. Obsessive-compulsive disorder Administer 20~60mg of this medication per day. The daily dose should not exceed 80mg. 4. Premenstrual dysphoric disorder Administer 20mg of this medication per day. Reassess the need for continued treatment after 6 months of treatment. Patients with renal and/or hepatic impairment, patients receiving multiple medications, elderly patients, and patients with comorbid diseases: Administer a lower dose or reduce the frequency of administration. Adjust the dose as appropriate according to age and symptoms. Switching from MAO inhibitors used to treat psychiatric disorders When discontinuing an MAO inhibitor used to treat a psychiatric disorder and starting treatment with this medication, allow an interval of at least 14 days. Conversely, at least 5 weeks must elapse after discontinuing this medication before starting an MAO inhibitor to treat a psychiatric disorder.
This medication may increase the risk of suicidal thoughts or suicidal behavior when used short-term in children, adolescents, and young adults aged 18 to 24 with major depression or psychiatric conditions. Therefore, the risks and benefits should be carefully evaluated when administering this medication to patients of this age group. This medication should not be used in patients with an allergic reaction, severe renal dysfunction, or those taking certain types of drugs such as MAO inhibitors. It should be used cautiously in patients with a history of seizures, and dosage should be discontinued if seizures occur or increase in frequency. Patients at risk of suicide should be particularly closely monitored at the beginning of treatment and when changing doses, and family or caregivers should also closely observe changes in the patient's condition. Patients with a history of mania or hypomania should also be monitored closely, as this medication can induce manic symptoms and should be discontinued if mania occurs. Patients with heart disease or those with a history of QT interval prolongation syndrome should perform an ECG and use this medication cautiously. If symptoms of cardiac arrhythmia develop during administration, discontinuation should be immediate. Patients with impaired liver or kidney function should have their dosage adjusted, and diabetic patients should periodically check their blood glucose levels, as this medication may alter glucose control. This medication can cause pupil dilation, so patients at risk for elevated intraocular pressure or glaucoma should be cautious. Common side effects during use include headache, nausea, insomnia, fatigue, diarrhea, etc., and rarely, allergic reactions or severe skin reactions may occur. Sudden discontinuation of this medication may result in withdrawal symptoms such as dizziness, sensory disturbances, sleep disorders, anxiety, nausea, tremors, and headaches, so the dose should be gradually reduced. Caution is required when taking this medication with other serotonin acting drugs, as it may lead to serious side effects known as serotonin syndrome. During pregnancy, it should only be used if necessary due to possible risks to the fetus, and during breastfeeding, consideration should be given to discontinuing breastfeeding, as the drug may be transmitted through breast milk. Safety and efficacy have not been established in children and adolescents, so it is not recommended. In the elderly, while no specific adverse reactions have been reported, caution is needed depending on other conditions or medications. During the administration of this medication, alcohol consumption should be avoided, and patients should be cautious when driving or operating machinery, and should consult their physician if taking other medications. It should be kept out of the reach of children, and changing the container should be avoided.
