Efficacy & effects

Improvement of blood glucose control in type 2 diabetes

View approved original text

This medication is used as an adjunct to help control blood glucose levels in patients with type 2 diabetes. Taking this medication alongside dietary control and exercise can make blood sugar management more effective. It can be used alone or, in some cases, in combination with other hypoglycemic agents or insulin. For example, if oral hypoglycemic agents are ineffective in controlling blood glucose, it may be administered with insulin, or in cases where control is difficult with sulfonylureas or metformin monotherapy, it may be given in combination with metformin. This helps to improve blood glucose control in various ways.

How to take

Adult dose

Individualized adjustments are made for each patient. Take with more than 1/2 cup of water without chewing. This medication is taken orally once a day. It should be taken before breakfast or just before the first meal. Start with a dose of 1 mg of Glimepiride once daily. If needed, the dose may be increased by 1 mg at intervals of 1 to 2 weeks. Generally, no additional efficacy is achieved at doses above 4 mg.

Precautions

This medication should not be used in patients with insulin-dependent diabetes, diabetic ketoacidosis, or coma. Patients who are allergic to this medication or its components, or to sulfonylureas, should not be administered this medication. There is no experience with this medication in patients with severe liver or kidney dysfunction, and in such cases, switching to insulin therapy is recommended. Pregnant women, women who may become pregnant, and nursing mothers should also refrain from taking this medication. It is advisable to apply insulin therapy for patients with severe infections, especially before and after surgery or for those who have sustained major trauma. Patients with gastrointestinal disturbances such as diarrhea or vomiting should be cautious as impaired food absorption increases the risk of hypoglycemia. Additionally, since this medication contains lactose, it is not given to patients with genetic issues like lactose intolerance or galactose intolerance. During the first week of taking this medication, the risk of hypoglycemia is heightened, so careful monitoring of blood glucose levels is particularly important. Patients or situations at high risk for hypoglycemia include elderly individuals who may be less cooperative, those with poor nutritional status or irregular eating habits, excessive exercise or changes in food intake, and those who consume alcohol. Patients with impaired kidney or liver function may also react more sensitively to this medication, so adjustments may be necessary. Additionally, caution is needed for patients with thyroid, pituitary, or adrenal dysfunctions, the elderly, and those on other medications. In these cases, blood glucose should be checked frequently, and if hypoglycemia symptoms occur, the doctor or pharmacist should be informed, and dosage or administration methods should be adjusted as necessary. This medication contains the dye Yellow No. 5, so patients allergic to this ingredient should use it with caution. The most important side effect to be cautious of while taking this medication is hypoglycemia. When hypoglycemia occurs, various symptoms such as headache, severe hunger, nausea, fatigue, anxiety, reduced concentration, confusion, dizziness, drowsiness, and loss of consciousness can manifest. In severe cases, it may lead to seizures or coma, requiring immediate treatment. Additionally, during the initial period of medication, temporary blurring of vision may occur due to blood sugar changes, and gastrointestinal disturbances, liver function abnormalities, blood anomalies, and allergic reactions may rarely occur. In particular, if allergic symptoms such as hives or difficulty breathing appear, the physician should be notified immediately, and in severe cases, medication should be discontinued. When taking this medication, it is important to take it regularly and to adhere to dietary and exercise regimens. If hypoglycemia occurs, consuming sugar or sugary drinks can quickly alleviate symptoms, so it is advisable to always carry a source of sugar. During the course of taking this medication, regular testing of blood glucose and urine is recommended, and if the effect is insufficient, other treatment options may be considered. Also, caution should be exercised when driving or operating machinery due to potential decreased alertness from hypoglycemia or hyperglycemia. This medication can interact with other drugs, so it is essential to inform the doctor or pharmacist when starting or stopping other medications. Especially if taken with drugs metabolized by the liver or drugs that raise or lower blood glucose levels, this can affect blood sugar control. Medications like beta-blockers can mask the symptoms of hypoglycemia, requiring caution. Moreover, alcohol consumption can unpredictably change the effects of this medication. Pregnant or nursing women are advised against taking this medication as it may affect the fetus or newborn. If necessary, switching to insulin therapy or stopping breastfeeding is recommended. The safety and efficacy of this medication in children under 18 have not been established, so it is best not to use it in this population. Elderly patients may have reduced kidney function, so adjusting the dosage of this medication may be necessary. In the event of an overdose of this medication, hypoglycemia symptoms may occur, so immediate medical treatment is required. Initially, emesis may be induced and adsorbent medications administered, and if necessary, glucose can be administered intravenously to regulate blood sugar levels. Symptoms of hypoglycemia may recur even after treatment, so careful observation for 24 to 48 hours is necessary. This medication should be stored out of reach of children and kept sealed in its original container to ensure safety.
