Treatment of major depressive disorder, anxiety disorders (including panic disorder and social anxiety disorder), and obsessive-compulsive disorder
This medication is used for the treatment of major depressive disorder, panic disorder, social anxiety disorder, generalized anxiety disorder, and obsessive-compulsive disorder. These conditions significantly impact mental health, and medication can help alleviate symptoms and improve quality of daily life.
The safety of doses exceeding 20mg daily has not been established. Administer this medicine once daily in the morning or evening, with or without food. Major depressive disorder: The usual dose is 10mg once daily. The dose may be increased to a maximum of 20mg daily according to the patient's response. If the dose is to be increased to 20mg, increase it after an interval of at least 1 week. Antidepressant effects generally require 2-4 weeks to develop. Even after symptoms resolve, at least 6 months of drug therapy is required to ensure the treatment effect. Panic disorder with or without agoraphobia: The recommended initial dose is 5mg daily for 1 week, followed by an increase to 10mg daily. The dose may be increased to a maximum of 20mg daily according to the patient's response. The maximum effect appears after approximately 3 months. The safety and efficacy of long-term administration of this medicine for more than 10 weeks in the treatment of panic disorder have not been systematically studied. The physician should periodically reevaluate each patient to determine the need for continued treatment. Social anxiety disorder: The usual dose is 10mg once daily. Improvement in symptoms generally requires 2-4 weeks. Thereafter, the dose may be reduced to 5mg daily or increased to a maximum of 20mg daily according to the patient's response. Social anxiety disorder is a chronic condition, and continued treatment for 12 weeks is recommended to ensure the treatment effect. Long-term treatment in responding patients has been studied for 6 months; consider treatment individually to prevent relapse. Periodically reevaluate the usefulness of long-term treatment. Generalized anxiety disorder: The initial dose is 10mg once daily. Depending on the patient's response, the dose may be increased to a maximum of 20mg daily after at least 1 week of treatment. A long-term study administered 20mg daily for at least 6 months to patients who responded to 12 weeks of open-label treatment. The physician should periodically reevaluate the usefulness of long-term treatment with this medicine for each patient. Obsessive-compulsive disorder: The usual dose is 10mg once daily. The dose may be increased to a maximum of 20mg daily according to the patient's response. A long-term study administered 10mg or 20mg daily for at least 24 weeks to patients who responded to 16 weeks of open-label treatment. Because obsessive-compulsive disorder is a chronic disease, treatment should continue for a sufficient period to ensure symptom resolution. If this medicine is used long term, regularly reevaluate its benefit and dose for each patient. No dose adjustment is necessary in patients with mild or moderate renal impairment. Use caution in patients with severe renal impairment (CLCR < 30㎖/min). Patients with hepatic impairment: The recommended initial dose is 5mg daily for the first 2 weeks. The dose may be increased to 10mg daily according to the patient's response. If treatment with this medicine is discontinued, gradually reduce the dose over at least 1-2 weeks to avoid the possibility of withdrawal symptoms. When discontinuing an MAO inhibitor used to treat psychiatric disorders and starting treatment with this medicine, allow an interval of at least 14 days. Conversely, when starting an MAO inhibitor to treat psychiatric disorders, at least 14 days must have elapsed after discontinuing this medicine.
This medication is an antidepressant used for treating psychiatric conditions such as major depression. It may increase the risk of suicidal thoughts and behaviors in children, adolescents, and young adults, so it should be prescribed with caution for patients in this age group. This drug should not be taken with certain medications, particularly MAO inhibitors, pizotifen, or drugs that prolong the QT interval. Patients with glaucoma or a history of glaucoma should use this medication with caution, as it can dilate the pupils and increase intraocular pressure. During the first 1-2 weeks, various side effects such as anxiety, insomnia, dizziness, and nausea may occur, but these often improve over time. This medication can cause mental changes such as worsening depression symptoms, suicidal thoughts, and aggression, so patients and caregivers should closely monitor for these symptoms. Abruptly discontinuing this medication can lead to withdrawal symptoms including dizziness, anxiety, and sleep disturbances, so it should be tapered off gradually under a doctor's guidance. Patients with heart disease or prolonged QT intervals should undergo an ECG prior to starting this medication and have their cardiac condition monitored regularly while on treatment. Pregnant or breastfeeding women should consult a physician fully regarding the use of this medication, and discontinuation of breastfeeding is recommended while on this drug. It is advisable not to consume alcohol while taking this medication, and caution is needed when driving or operating machinery. The safety and efficacy of this medication have not been established for children and adolescents under 18 years of age, hence it is preferable not to use it in this population. Older adults should start at a lower dose, and overall monitoring of the patient's condition is crucial during treatment.
