Monotherapy for partial-onset seizures and adjunctive treatment of epileptic seizures (partial-onset, myoclonic, etc.)
This medication is used for the treatment of epilepsy. It can be used as monotherapy for patients aged 4 years and older diagnosed with secondary generalized seizures or partial seizures as the first diagnosis. Additionally, it is administered as adjunct therapy for secondary generalized seizures or partial seizures that are difficult to control with existing first-line therapies. It is also effective for treating myoclonic seizures and generalized tonic-clonic epilepsy in pediatric patients over 12 years of age. In particular, it can be administered as an injection for patients who cannot take oral medications. This medication effectively treats various types of seizures.
The film-coated tablets and oral solution are administered orally and may be taken with or without food, with a sufficient amount of water. The total daily dose is divided into 2 equal doses administered twice daily. For monotherapy and adjunctive therapy of partial-onset seizures, adults (18 years and older) and adolescents weighing 50 kg or more (12~17 years) should start with 500mg twice daily. Depending on clinical response, the dose may be increased by 1000mg/day every 2 to 4 weeks, up to the maximum recommended dose of 3000mg/day. For adjunctive therapy of primary generalized tonic-clonic seizures, adults and adolescents weighing 50 kg or more should start with 500mg twice daily (1000mg/day). The dose is increased by 1000mg/day every 2 weeks, up to a maximum of 3000mg/day. In patients with renal impairment, adjust the twice-daily dose according to creatinine clearance. For patients with end-stage renal disease undergoing dialysis, once-daily administration is recommended. Dose adjustment according to renal function is also recommended in older adults (65 years and older).
Children 4~11 years of age and adolescents 12~17 years of age weighing less than 50 kg should start with 10mg/kg twice daily. Depending on clinical response, the dose may be increased to 60mg/kg/day; do not increase or decrease the daily dose by more than 20mg/kg every 2 weeks. For children weighing 20 kg or less, treatment initiation with the 100mg/ml oral solution is recommended. Children weighing 20~40kg should start with 250mg twice daily (500mg/day). The dose may be increased by 500mg/day every 2 weeks, up to a maximum of 1500mg/day. Children weighing more than 40kg should start with 500mg twice daily (1000mg/day). The dose may be increased by 1000mg/day every 2 weeks, up to a maximum of 3000mg/day. For adjunctive therapy of myoclonic seizures (12 years and older), start with 500mg twice daily. The dose is increased by 1000mg/day every 2 weeks, up to a maximum of 3000mg/day. For adjunctive therapy of primary generalized tonic-clonic seizures, adolescents 12~17 years of age weighing less than 50 kg should start with 10mg/kg twice daily. The dose is increased by 20mg/kg/day every 2 weeks, up to a maximum of 60mg/kg/day. Children and adolescents weighing less than 50 kg with renal impairment also require adjustment of the twice-daily dose according to creatinine clearance. For patients with end-stage renal disease undergoing dialysis, once-daily administration is recommended. Use is not recommended in infants younger than 1 month. Tablets are not suitable for children 4 years of age or younger; the oral solution should be used. The injection is not recommended for children under 16 years of age.
This medication is used for treating epilepsy, but there is an increased risk of suicidal thoughts or behaviors during treatment. Patients and caregivers should carefully monitor for mood changes or symptoms of depression. Additionally, this drug may cause neuropsychiatric adverse reactions such as drowsiness, fatigue, impaired coordination, and behavioral abnormalities, particularly during the initial stages of treatment. Abrupt discontinuation of the medication may worsen seizures, so the dosage should be gradually reduced according to a doctor's instructions. Patients allergic to any component of this drug should not take it, and those with impaired kidney or liver function may require dosage adjustments. Since there is a possibility of decreased blood cell counts, if symptoms such as fever or weakness appear, immediate consultation with a doctor is necessary. Neuropsychiatric symptoms such as irritability, aggressiveness, depression, and anxiety may occur, and if these symptoms are severe, consideration should be given to medication adjustment or discontinuation. This drug may also worsen seizures, so if any abnormal symptoms arise, consultation with medical personnel should be sought immediately. There have been rare reports of QT interval prolongation in patients with heart-related issues, so caution is needed for patients with heart disease. The effects on the fetus during pregnancy are not fully known, and women planning to become pregnant or pregnant should definitely consult their healthcare provider. In breastfeeding cases, the medication may pass through breast milk, necessitating reconsideration of medication use or stopping breastfeeding. Safety and efficacy have not been established for children, especially infants under 1 month old, and children under 4 years should use liquid formulations instead of tablets. Elderly patients may have an increased risk of adverse effects due to decreased kidney function, requiring dosage adjustments. Caution is needed while driving or operating machinery as drowsiness or decreased concentration may occur. In the event of overdose, symptoms such as drowsiness, aggression, and reduced consciousness may appear. As there is no specific antidote, it is important to receive treatment according to the symptoms. This medication has relatively few interactions with other drugs, but when used in conjunction with specific medications, monitoring of blood concentration changes is required.
