Adjunctive treatment of parkinsonian syndromes that do not improve with standard therapy
This medication is used as an adjunctive therapy for patients with Parkinson's syndrome whose symptoms do not improve with standard therapy using levodopa and dopa decarboxylase inhibitors. In other words, it contributes to improving the patient's motor function by helping alleviate symptoms when the primary treatment is not sufficiently effective.
This drug should be administered orally at the same time as each dose of levodopa/dopa decarboxylase inhibitor (benserazide or carbidopa). This drug may be used with standard levodopa formulations. The efficacy of this drug as an adjunct to sustained-release levodopa/dopa decarboxylase inhibitor formulations has not been established. This drug may be administered with or without food. Administer 1 tablet of this drug 200mg with each dose of levodopa/dopa decarboxylase inhibitor. The maximum recommended dose of this drug is 200mg per dose 10 times daily, i.e., 2000mg. This drug enhances the effect of levodopa. Therefore, to reduce levodopa-related dopaminergic adverse effects, such as dyskinesia, nausea, vomiting, and hallucinations, it may occasionally be necessary to adjust the levodopa dose within the first 1 day to 1 week of starting this drug. The daily levodopa dose should be reduced by approximately 10 ~ 30% by extending the dosing interval or reducing the dose per administration, depending on the patient's clinical condition. When discontinuing this drug, it may be necessary to adjust the dose of other antiparkinsonian drugs, particularly levodopa, to control Parkinson's symptoms. This drug increases the bioavailability of levodopa in standard levodopa/benserazide formulations slightly more (5~10%) than that of levodopa in standard levodopa/carbidopa formulations. Therefore, patients receiving standard levodopa/benserazide formulations may require a greater reduction in the levodopa dose when starting this drug. Renal failure does not affect the pharmacokinetics of this drug; therefore, no dose adjustment is required. However, extending the dosing interval may be considered in patients on dialysis. Older adults: No dose adjustment of this drug is required in older adults.
Studies have not been conducted in patients 18 years of age or younger; therefore, use in patients in this age group is not recommended.
This drug should not be used in patients with allergies to the active ingredient or other components. It should also not be taken by women who are pregnant or nursing. It is not suitable for patients with impaired liver function. Patients with certain rare tumors known as pheochromocytoma have a risk of sudden blood pressure elevation when taking this drug. It should not be taken with certain types of antidepressants or monoamine oxidase inhibitors, and dosage restrictions are necessary for some medications. Patients with a history of neuroleptic malignant syndrome or rhabdomyolysis should exercise caution. This drug may increase dopaminergic effects, potentially causing side effects such as movement disorders, nausea, vomiting, abdominal pain, diarrhea, etc., which may occur more frequently during the early stages of treatment. Urine may turn reddish-brown, but this is a harmless phenomenon. Prolonged use may slightly reduce hemoglobin levels in the blood, necessitating regular testing. Liver enzyme levels may rarely rise, so liver function tests are recommended. Dosage adjustment may be necessary when taking this drug with levodopa, as symptoms of orthostatic hypotension may appear, so caution is required. Care should be taken while driving or operating machinery, as drowsiness or dizziness may occur. If diarrhea persists, weight should be regularly monitored to prevent weight loss, and consultation with a doctor is recommended if necessary. Impulse control disorders may occur, so patients and caregivers should closely observe for behavioral changes such as pathological gambling and increased libido. This drug should not be administered to pregnant or nursing women and is not recommended for children under 18 years old. In case of overdose, symptoms such as confusion, drowsiness, and reduced muscle tone may occur, necessitating immediate medical assistance. This drug may interact with certain medications, so it is essential to inform healthcare providers about any other medications being taken. It is recommended to take iron supplements 2 to 3 hours apart before and after taking this medication. It is important to immediately consult a healthcare professional if any unusual symptoms arise while taking this drug.
