Prevention of thrombotic cardiovascular events (such as myocardial infarction and stroke)
This medication is used to prevent thrombotic cardiovascular events. It is particularly used in patients with acute coronary syndrome or a history of myocardial infarction, with a high risk of thrombotic cardiovascular events, and is used in conjunction with aspirin to reduce mortality due to cardiovascular abnormalities and the incidence of myocardial infarction or stroke. Additionally, in patients with type 2 diabetes who have undergone percutaneous coronary intervention without any history of myocardial infarction or stroke, this medication is used in combination with aspirin to reduce the occurrence of myocardial infarction and stroke. It also helps to reduce the risk of stroke in patients with acute ischemic stroke or high-risk transient ischemic attacks when used with aspirin. Thus, in patients with various risk factors for thrombotic cardiovascular events, this medication plays an important preventive role in improving cardiovascular health.
This medicine should not be co-administered with other ADP receptor inhibitors (ticlopidine, clopidogrel, prasugrel). Acute coronary syndrome: Administer an initial dose of 180mg, followed by 90mg twice daily. Treatment for 12 months is recommended unless discontinuation of this medicine is clinically necessary. Patients taking this medicine should receive a maintenance dose of aspirin (75 - 150mg) concomitantly unless specifically contraindicated. In patients with acute coronary syndrome who are at increased risk of bleeding after percutaneous coronary intervention (PCI), monotherapy with this medicine (discontinuation of aspirin) may be considered after 3 months of concomitant treatment with this medicine and aspirin. (See 3. Patients requiring careful administration.) Patients with a history of myocardial infarction (occurring at least 1 year previously) who are at high risk of thrombotic cardiovascular events: Administer 60mg twice daily. Patients taking this medicine should receive a maintenance dose of aspirin (75 - 150mg) concomitantly unless specifically contraindicated. Patients who were taking this medicine 90mg or another ADP receptor inhibitor (ticlopidine, clopidogrel, prasugrel) during the first year after myocardial infarction may continue treatment with this medicine 60mg. Treatment with this medicine may also be initiated in patients within 2 years after myocardial infarction or within 1 year after discontinuation of a previous ADP receptor inhibitor (ticlopidine, clopidogrel, prasugrel). The safety and efficacy of this medicine were evaluated at 3 years of follow-up. Patients with type 2 diabetes who have undergone percutaneous coronary intervention (PCI) for coronary artery disease: Administer 60mg twice daily. Patients taking this medicine should receive a maintenance dose of aspirin (75 - 150mg) concomitantly unless specifically contraindicated. For patients with acute coronary syndrome, refer to the corresponding dosage and administration instructions. Data on the safety and efficacy of this medicine for treatment longer than 3 years are limited. Patients with acute ischemic stroke or high-risk transient ischemic attack: Administer an initial dose of 180mg, followed by 90mg twice daily for 30 days. Patients taking this medicine should receive a maintenance dose of aspirin (75 - 100mg) concomitantly unless specifically contraindicated. An initial loading dose of aspirin (300-325mg) is recommended in patients with acute ischemic stroke or high-risk transient ischemic attack. This medicine is administered orally and may be taken with or without food. For patients who have difficulty swallowing tablets, crush this medicine into a fine powder, disperse it in half a glass of water, and drink it immediately; rinse the glass with another half glass of water and drink that as well. This mixture may also be administered through a nasogastric tube (CH8 or larger); it is important to flush the nasogastric tube with water after administration. When switching from another antiplatelet agent to this medicine, start this medicine 24 hours after the last dose of the previous medicine. If a dose is missed, take only 1 tablet (the next scheduled dose) at the next scheduled dosing time. No dose adjustment is required in patients with renal impairment. No dose adjustment is required in patients with mild hepatic impairment. This medicine has not been studied in patients with severe hepatic impairment, and experience with its use in patients with moderate hepatic impairment is limited.
The safety and efficacy have not been established in pediatric patients under 18 years (see 12. Information for healthcare professionals).
This medication prevents blood from becoming excessively thick to avoid clot formation. However, taking this medication may increase the risk of bleeding; thus, it should not be used in patients with bleeding disorders or a history of intracranial bleeding. If surgery is planned, the medication should be discontinued at least 5 days prior to the procedure, and any symptoms of bleeding should be reported to healthcare providers immediately. It should be avoided in patients with a known allergy to this medication, severely impaired liver function, or those taking certain strong medications. Caution should be exercised in patients with a high risk of bleeding, recent trauma, surgery, or gastrointestinal bleeding when using this medication, and interactions with other drugs affecting blood coagulation should be carefully monitored. Patients may experience shortness of breath or difficulty breathing during treatment, so those with a history of respiratory diseases should take special care. Furthermore, since there is a risk of slow heart rate, patients with a history of heart disease or symptoms of bradycardia should consult with their healthcare provider. Women who are pregnant or breastfeeding must consult with their healthcare provider before taking this medication, and women of childbearing potential should use appropriate contraception to avoid pregnancy. Excessive doses may increase the risk of bleeding, and there is no specific antidote, so if bleeding occurs, appropriate treatment should be sought immediately.
