Treatment for hypercholesterolemia (primary, familial) and reduction of cardiovascular disease risk
This medication is an adjunct used when dietary management and exercise are insufficient for controlling blood lipid levels in conditions such as primary hypercholesterolemia and combined hyperlipidemia. It is particularly helpful in lowering total cholesterol and LDL cholesterol levels in patients with heterozygous familial hypercholesterolemia and hypercholesterolemia, thereby delaying the progression of atherosclerosis. It is also used as a dietary adjunct in pediatric patients aged 10 to 17 with certain conditions. Additionally, it is beneficial for patients with primary dysbetalipoproteinemia. This medication contributes to reducing the risk of stroke, myocardial infarction, and arterial revascularization procedures in elderly adults at high risk for cardiovascular disease. Overall, it is used to improve blood lipid levels and reduce the risk of cardiovascular diseases.
The initial dosage is 5 mg once daily. If a greater reduction in LDL cholesterol is needed, the maintenance dosage can be adjusted accordingly. The maintenance dosage is typically 10 mg once daily, with most patients adjusted at this dosage. The maintenance dosage should be appropriately adjusted based on LDL cholesterol levels, treatment goals, and patient response, with a maximum of 20 mg once daily allowed.
For pediatric patients with heterozygous familial hypercholesterolemia (ages 10 to 17), the usual dosage range is 5 mg to 20 mg once daily. The dosage should be tailored to meet the individual treatment goals of each patient.
This medication should not be used in certain patients. For example, individuals allergic to any of the components of this medication, those with severe liver dysfunction, and those with muscle diseases should not take it. Also, those with significantly impaired kidney function or women who are pregnant or breastfeeding may be at risk if they take this medication. In particular, individuals prone to muscle diseases should avoid high doses, and those with genetic issues such as lactose intolerance should also refrain from taking it. Before taking this medication, it is essential to check liver function and muscle condition. Those who consume excessive alcohol or have a history of liver disease should be cautious, and treatment of the underlying cause should be done before administering the medication to individuals with hypothyroidism or nephrotic syndrome. If symptoms such as muscle pain, weakness, or cramps occur during treatment, inform a physician immediately, and the medication may need to be discontinued. In cases of acute illness or severe conditions, discontinuation of this medication should be considered, and immediate cessation is required if severe muscle weakness or ophthalmoplegia occurs. Additionally, caution is needed as the risk of muscle side effects may increase when certain other medications are used concurrently. This medication may cause proteinuria in blood tests, though this is mostly temporary and does not lead to severe renal failure. However, it is advisable to undergo regular kidney function tests during high-dose therapy. The concentration of this medication in the body may be higher in Asians, so dosage adjustments may be necessary. Rarely, side effects such as headaches, dizziness, dyspepsia, skin rashes, myalgia may occur during therapy. If severe skin reactions or liver damage symptoms appear, discontinue use immediately and consult healthcare personnel. The safety of this medication during pregnancy and lactation has not been established, so it should be avoided. Women who could become pregnant should use appropriate contraception. In case of overdose, there is no specific antidote, so treatment should be based on symptoms, along with checking liver function and muscle enzyme levels. Caution is needed as dizziness may occur when driving or operating machinery. Lastly, this medication can interact with several other drugs, so inform your physician or pharmacist if you are on other medications.
Hives, severe swelling (edema), chest tightness, difficulty breathing, thrombocytopenia, jaundice, hepatitis, Stevens-Johnson syndrome, eosinophilia with systemic symptoms drug reaction (DRESS), immune-mediated necrotizing myopathy, rhabdomyolysis, acute kidney failure, oliguria, increased serum creatinine, severe muscle weakness, ophthalmoplegia, severe allergic reactions, severe skin reactions, respiratory problems, liver abnormalities (jaundice).

This medication is sensitive to air and moisture, so it should be stored in a tightly closed container. It is best to keep it at room temperature between 1°C and 30°C, away from extreme heat or cold.
