Treatment of multiple myeloma, anemia in myelodysplastic syndromes, and lymphoma (mantle cell and follicular)
This medication enhances therapeutic effects when used in combination with dexamethasone or melphalan, prednisolone in patients with multiple myeloma. It is applicable to newly diagnosed patients as well as previously treated patients. Additionally, it is used as maintenance therapy in patients who have undergone autologous stem cell transplantation to prevent relapse. It is also helpful in treating transfusion-dependent anemia in patients with low-risk or intermediate-1 risk myelodysplastic syndromes with 5q deletion. In patients with mantle cell lymphoma and follicular lymphoma, it improves therapeutic effect when used in combination with rituximab. This medication is widely used in the treatment of hematologic cancers and related disorders.
Administer only to patients who have agreed to comply with the requirements specified in the Pregnancy Prevention Program. Women of childbearing potential must have negative pregnancy test results confirmed continuously according to the specified schedule. 1. Multiple myeloma 1) Patients who have received at least one prior therapy: Treatment must not be initiated if the absolute neutrophil count is <1,000/㎕ and/or the platelet count is <75,000/㎕. The recommended initial dose is 25mg once daily. Take 25mg capsules on days 1–21 of each 28-day cycle. During the first 4 cycles, administer dexamethasone 40mg once daily on days 1–4, 9–12, and 17–20 of each 28-day cycle. After 4 cycles, administer 40mg once daily only on days 1–4 of each 28-day cycle. Dose adjustment according to platelet count: If the count decreases to <30,000/㎕, interrupt treatment and monitor a complete blood count weekly. When it recovers to ≧30,000/㎕, resume treatment at 15mg once daily. Thereafter, interrupt treatment whenever the count decreases to <30,000/㎕. When it recovers to ≧30,000/㎕, resume treatment at a dose reduced by 5mg from the dose being taken. Do not reduce the dose below 5mg daily. Dose adjustment according to absolute neutrophil count (ANC): If the count decreases to <1,000/㎕, interrupt treatment, add G-CSF, and monitor a complete blood count weekly. When it recovers to ≧1,000/㎕ and neutropenia is the only toxicity, resume treatment at 25mg once daily. When it recovers to ≧1,000/㎕ and other toxicities have occurred, resume treatment at 15mg once daily. Thereafter, interrupt treatment whenever the count decreases to <1,000/㎕. When it recovers to ≧1,000/㎕, resume treatment at a dose reduced by 5mg from the dose being taken. Do not reduce the dose below 5mg daily. 2) Newly diagnosed patients who are not eligible for transplantation: Treatment must not be initiated if the absolute neutrophil count is <1,000/㎕ and/or the platelet count is <50,000/㎕. The recommended initial dose is 25mg once daily. Take 25 mg capsules on days 1–21 of each 28-day cycle. Administer dexamethasone 40mg once daily on days 1, 8, 15, and 22 of each 28-day cycle. For patients aged 75 years or older, administer 20mg once daily on the same days. Continue until disease progression or unacceptable toxicity occurs. This drug can cause severe fetal malformations or abortion if taken during pregnancy, so women who are pregnant or may become pregnant should not take it. Women of childbearing potential must have two pregnancy tests before taking this drug and use appropriate contraceptive methods for a certain period both before and after treatment. This medication can reduce the number of white blood cells and platelets in the blood; therefore, regular blood tests should be done to check blood cell counts, and symptoms of infection or bleeding should be reported to a healthcare provider immediately. Caution is required for heart and vascular health due to the risk of myocardial infarction or thrombosis, so patients with cardiovascular risk factors need careful observation and management. This drug can affect liver function; therefore, patients with liver disease or abnormal liver function test results should have their liver function monitored regularly. Patients with reduced kidney function need dosage adjustments, and it is important to regularly check their kidney function. Tumor lysis syndrome and tumor flare reaction can occur, so high-risk patients should be monitored closely and take appropriate precautions at the start of treatment and when dose increases occur. There may be an increased risk of infection, so if any symptoms of infection occur, promptly visit a medical institution for appropriate treatment. While taking this medication, serious adverse reactions such as severe skin reactions, anaphylaxis, pulmonary hypertension may occur, and if any unusual symptoms develop, consult a doctor immediately. This drug should not be administered to patients with certain genetic disorders (such as galactose intolerance) and should be avoided by patients who have experienced hypersensitivity reactions. While on treatment, various adverse reactions should be regularly monitored, and immediate actions should be taken if any unusual symptoms appear. This medication requires professional clinical management and monitoring, so it should be taken exactly as prescribed by a physician, along with regular check-ups.
