Treatment of major depressive disorder, anxiety disorders (including panic disorder and social anxiety disorder), and obsessive-compulsive disorder
This medicine is used to treat various mental health issues, such as major depressive disorder, panic disorder with or without agoraphobia, social anxiety disorder (social phobia), generalized anxiety disorder, and obsessive-compulsive disorder.
Safety has not been established for doses exceeding 20 mg daily. This medicine is administered once daily, in the morning or evening, regardless of meals. Major depressive disorder: The usual dose is 10 mg once daily. The dose may be increased to a maximum of 20 mg daily depending on the patient's response. If increasing the dose to 20 mg, allow an interval of at least 1 week before increasing it. Antidepressant effects generally take 2-4 weeks to develop. Even after symptoms resolve, drug treatment for at least 6 months is required to ensure the treatment effect. Panic disorder with or without agoraphobia: The recommended starting dose is 5 mg daily for 1 week, followed by an increase to 10 mg daily. The dose may be increased to a maximum of 20 mg daily depending on the patient's response. The maximum effect occurs after approximately 3 months. The safety and efficacy of long-term administration of this medicine for more than 10 weeks in the treatment of panic disorder have not been systematically studied. Social anxiety disorder: The usual dose is 10 mg once daily. Symptom improvement generally takes 2-4 weeks. Thereafter, depending on the patient's response, the dose may be reduced to 5 mg daily or increased to a maximum of 20 mg daily. To ensure the treatment effect, continuing treatment for 12 weeks is recommended. Long-term treatment has been studied for 6 months. Generalized anxiety disorder: The starting dose is 10 mg once daily. Depending on the patient's response, the dose may be increased to a maximum of 20 mg daily after at least 1 week of treatment. A long-term treatment trial administering 20 mg daily for at least 6 months was conducted. Obsessive-compulsive disorder: The usual dose is 10 mg once daily. Depending on the patient's response, the dose may be increased to a maximum of 20 mg daily. Long-term treatment trials administering 10 mg or 20 mg daily for at least 24 weeks were conducted. Patients with renal impairment: No dose adjustment is necessary in patients with mild or moderate renal impairment. Use with caution in patients with severe renal impairment (CLCR < 30 mL/min). Patients with hepatic impairment: The recommended starting dose is 5 mg daily for the first 2 weeks. Depending on the patient's response, the dose may be increased to 10 mg daily. Discontinuation: When discontinuing this medicine, gradually reduce the dose over at least 1-2 weeks to avoid the possibility of withdrawal symptoms. Switching from MAO inhibitors for the treatment of psychiatric disorders: When discontinuing an MAO inhibitor used to treat psychiatric disorders and starting treatment with this medicine, allow an interval of at least 14 days. Conversely, when starting an MAO inhibitor to treat psychiatric disorders, at least 14 days must have elapsed since discontinuing this medicine. Other MAO inhibitors, such as linezolid or methylene blue: Patients receiving linezolid or intravenous methylene blue must not start treatment with this medicine. If linezolid or intravenous methylene blue must be administered urgently to a patient already receiving this medicine, no alternative treatment is available, and the benefits of treatment are considered to outweigh the risks, this medicine may be discontinued immediately and linezolid or intravenous methylene blue administered. Monitor the patient for symptoms of serotonin syndrome for 2 weeks after administration of linezolid or intravenous methylene blue, or until 24 hours after the last dose, whichever occurs first. Treatment with this medicine may be restarted 24 hours after the last dose of linezolid or intravenous methylene blue. The risk associated with administration of methylene blue by non-intravenous routes (oral tablets or local injection) or with intravenous doses of 1 mg/kg or less is unclear.
This medication is an antidepressant used in the treatment of major depression and other psychiatric disorders. During treatment, especially in children, adolescents, and young adults aged 18 to 24, there may be an increased risk of suicidal thoughts or behavior; therefore, physicians should carefully assess the need and risks of treatment. This medication should not be taken with certain drugs, particularly MAO inhibitors or pimozide, as it can be dangerous. Patients with a history of glaucoma or those with narrow-angle glaucoma must use this medication with caution, as pupil dilation can increase intraocular pressure. In the first 1-2 weeks of treatment, side effects such as anxiety, insomnia, dizziness, and nausea may commonly appear but usually decrease over time. This medication can affect the heart, leading to QT interval prolongation or arrhythmias, so patients with heart conditions should consult their doctor before taking it. Additionally, stopping the medication suddenly can lead to withdrawal symptoms such as dizziness, anxiety, and sleep disturbances; thus, doses should be gradually reduced according to the physician’s instructions. Pregnant or breastfeeding women should consult a specialist before taking this medication, as it can affect the newborn. It is advisable not to take this medication with alcohol, and caution is necessary when driving or operating machinery. Due to many potential drug interactions, patients should inform their physician or pharmacist of any other medications they are taking. This medication has not been established as safe for use in children and adolescents. Elderly patients should start at a lower dose and be closely monitored. If there are changes in mental state, suicidal thoughts, or aggressive behavior while taking this medication, consult healthcare professionals immediately.
