Treatment of major depressive disorder, anxiety disorders (panic disorder, social anxiety, etc.), and obsessive-compulsive disorder
This medication is used to treat various mental health issues such as major depressive disorder, panic disorder, social anxiety disorder (social phobia), generalized anxiety disorder, and obsessive-compulsive disorder. It helps alleviate different anxiety and depressive symptoms, thus improving the daily lives of patients. It reduces mental tension and anxiety, while promoting mental health stability. This medication effectively manages the symptoms of these mental disorders, contributing to an improved quality of life. Therefore, it is used for several anxiety-related disorders, including major depressive disorder and panic disorder.
Safety has not been established for doses exceeding 20mg per day. This medication is administered once daily in the morning or evening, with or without food. 1) Adults (1) Major depressive disorder: The usual dose is 10mg once daily. The dose may be increased to a maximum of 20mg per day, depending on the patient's response. If the dose is to be increased to 20mg, it should be increased at intervals of at least 1 week. In general, 2-4 weeks are needed for antidepressants to take effect. Even after symptoms have resolved, medication treatment for at least 6 months is needed to ensure the therapeutic effect. (2) Panic disorder with or without agoraphobia: It is recommended to start with 5mg per day for 1 week, then increase to 10mg per day. The dose may be increased to a maximum of 20mg per day, depending on the patient's response. The maximum effect appears after approximately 3 months. The safety and efficacy of long-term administration of this medication for more than 10 weeks in the treatment of panic disorder have not been systematically studied. The physician should periodically reassess each patient to determine the need for continued treatment. (3) Social anxiety disorder: The usual dose is 10mg once daily. In general, 2-4 weeks are needed for symptoms to improve. Thereafter, the dose may be reduced to 5mg per day or increased to a maximum of 20mg per day, depending on the patient's response. Social anxiety disorder is a condition with a chronic course, and continued treatment for 12 weeks is recommended to ensure the therapeutic effect. Long-term treatment in patients who responded to treatment was studied for 6 months; it should be considered on an individual patient basis to prevent relapse. The usefulness of long-term treatment should be reassessed periodically. (4) Generalized anxiety disorder: The initial dose is 10mg once daily. Depending on the patient's response, the dose may be increased to a maximum of 20mg per day after at least 1 week of treatment. A long-term study was conducted in patients who responded to 12 weeks of open-label treatment, with 20mg per day administered for at least 6 months. The physician should periodically reassess the usefulness of long-term treatment with this medication for each patient. (5) Obsessive-compulsive disorder: The usual dose is 10mg once daily. The dose may be increased to a maximum of 20mg per day, depending on the patient's response. A long-term study was conducted in patients who responded to 16 weeks of open-label treatment, with 10mg or 20mg per day administered for at least 24 weeks. Obsessive-compulsive disorder is a chronic condition; treatment should be continued for a sufficient period to ensure recovery from symptoms. If this medication is administered long term, the usefulness and dosage should be periodically reassessed for each patient. 2) Patients with renal impairment No dosage adjustment is necessary in patients with mild or moderate renal impairment. Caution should be exercised in patients with severe renal impairment (CLCR < 30㎖/min). 3) Patients with hepatic impairment An initial dose of 5mg per day for the first 2 weeks is recommended. The dose may be increased to 10mg per day, depending on the patient's response. 4) Discontinuation of treatment When discontinuing this medication, the dose should be gradually reduced over at least 1-2 weeks to avoid the possibility of withdrawal symptoms. 5) Switching from MAO inhibitors for the treatment of psychiatric disorders When discontinuing an MAO inhibitor used to treat psychiatric disorders and starting treatment with this medication, an interval of at least 14 days should be allowed. Conversely, if starting an MAO inhibitor for the treatment of psychiatric disorders, at least 14 days must have elapsed after discontinuing this medication. 6) Other MAO inhibitors, such as linezolid or methylene blue Patients receiving linezolid or intravenous methylene blue should not start this medication because of the increased risk of serotonin syndrome. For patients who require treatment for a more urgent psychiatric condition, including hospitalization, other intervention procedures should be considered. It may be necessary to urgently administer linezolid or intravenous methylene blue to a patient already receiving this medication. If there is no alternative to linezolid or intravenous methylene blue and, for a particular patient, the benefits of treatment with linezolid or intravenous methylene blue are considered to outweigh the risk of serotonin syndrome, this medication may be stopped immediately and linezolid or intravenous methylene blue administered. Patients should be monitored for symptoms of serotonin syndrome for 2 weeks after administration of linezolid or intravenous methylene blue, or until 24 hours after the last dose, whichever occurs first. Treatment with this medication may be restarted 24 hours after the last dose of linezolid or intravenous methylene blue. The risks associated with administration of methylene blue by nonintravenous routes (oral tablets or local injection) or intravenous administration of this medication at a dose of 1mg/kg or less are unclear.
This medication is primarily used to treat symptoms associated with depression. Caution is required as this medication may increase the risk of suicidal thoughts or behaviors in children, adolescents, and young adults. Careful monitoring and physician judgment are necessary when administering this drug to patients in these age groups. This medication should not be taken with certain drugs, particularly MAO inhibitors, pimozide, and drugs that prolong the QT interval, as serious side effects may occur with such combinations. Patients with a history of glaucoma or those with closed-angle glaucoma should be cautious, as this medication may increase the risk of elevated intraocular pressure. During treatment, changes in mental state such as insomnia, anxiety, agitation, and suicidal thoughts may occur, so patients and caregivers should monitor closely, and if any unusual symptoms arise, consult a doctor immediately. Adverse reactions may be more common in the first 1-2 weeks after starting treatment, but they often decrease over time. This medication may cause a variety of side effects, including blood pressure changes, heart rhythm disturbances, gastrointestinal issues, skin rashes, muscle pain, and sexual dysfunction, and any severe or persistent symptoms should be reported to healthcare providers. Particularly, sudden discontinuation of this medication may lead to withdrawal symptoms such as dizziness, anxiety, and sleep disturbances; therefore, do not stop taking the medication abruptly and gradually reduce the dose as instructed by a physician. Women who are pregnant or breastfeeding should consult a physician before taking this medication, as it may pass into breast milk. Use during breastfeeding should be avoided or breastfeeding should be stopped. Elderly patients should start with a lower dose and be administered with caution, and those with heart disease or liver dysfunction should also be monitored closely during treatment. Caution is also warranted when driving or operating machinery, as this medication may affect mental or physical functions. In case of overdose, Central Nervous System symptoms, gastrointestinal disturbances, and cardiac arrhythmias may occur, necessitating immediate medical attention.
