Patients with hereditary angioedema or a history of angioedema while on ACE inhibitors or angiotensin II receptor antagonists should avoid this medication. Patients with severe renal impairment, primary hyperaldosteronism, and those with certain renal disorders and diabetes should avoid this medication in combination with specific drugs. Elderly patients or those with hyperkalemia, and patients with mild liver impairment should be especially cautious when using this medication, as they may experience a rapid drop in blood pressure; therefore, starting with a low dose and gradually increasing it is recommended. Patients with heart failure or after myocardial infarction should pay attention to changes in renal function and hypotension and must undergo renal function tests before and during treatment. This medication can increase potassium levels in the blood, so caution is necessary when taken with potassium-sparing diuretics or potassium supplements, and frequent monitoring of potassium levels is advised. When co-administered with other antihypertensives, diuretics, or NSAIDs, the effectiveness may change, hence consultation with a physician or pharmacist is required.
