Treatment of hypertension; adjunctive treatment of congestive heart failure; prevention of cardiovascular events in stable coronary artery disease
This drug controls blood pressure in patients with hypertension, reducing the burden on the cardiovascular system. It also assists in heart function when used with certain diuretics and digoxin in patients with congestive heart failure. In patients with stable coronary artery disease, it helps lower the risk of mortality or myocardial infarction due to cardiovascular diseases. Due to these effects, it is effective in maintaining cardiovascular health and preventing related complications. Therefore, it is used for hypertension, congestive heart failure, and stable coronary artery disease.
The dose should be adjusted according to the patient's condition and blood pressure response. It is recommended that this medicine be taken once daily in the morning before breakfast. 1. Hypertension 1) Adults: Administer 4 mg of perindopril tert-butylamine orally once daily in the morning. If necessary, the dose may be increased to 8 mg once daily 1 month after treatment is initiated. 2) Patients with highly activated renin-angiotensin-aldosterone system (RAAS) (especially those with renovascular hypertension, salt and/or fluid depletion, decompensated heart failure, or severe hypertension): Since an excessive decrease in blood pressure may occur after administration of 4 mg of this medicine, an initial dose of 2 mg is recommended for these patients under medical supervision. 3) Elderly patients: Depending on the patient's condition and severity, administer an initial dose of 2 mg once daily in the morning. If necessary, the dose may be increased to 4 mg once daily 1 month after treatment is initiated. 4) Patients with renal impairment: Adjust the dose according to the severity of renal impairment and administer the following doses based on measurements of serum potassium and creatinine. Creatinine clearance ≥ 60 mL/min: 4mg/day Creatinine clearance 30-60ml/min: 2mg/day Creatinine clearance 15-30ml/min: 2mg every other day Patients on hemodialysis* < 15ml/min: 2mg on dialysis days *The dialysis clearance of perindoprilat, the active metabolite of this medicine, is 70 mL/min. Patients undergoing hemodialysis should take this medicine after dialysis. 5) Patients taking diuretics When initiating treatment with 4 mg of this medicine, symptomatic hypotension may occur, primarily in patients taking diuretics. These patients may have salt and/or fluid depletion; therefore, administer with caution. If possible, discontinue the diuretic 2 ∼ 3 days before starting treatment with 4 mg of this medicine. For patients in whom discontinuation of the diuretic is not possible, initiate treatment with 2 mg of this medicine. In this case, monitor renal function and serum potassium, and adjust the dose of this medicine according to the blood pressure response. Diuretic treatment may be resumed if necessary. 6) Renovascular hypertension: Revascularization is preferred; however, if treatment is given before surgery or surgery cannot be performed, administer an initial dose of 2 mg once daily with caution. Continue to monitor renal function during treatment. 7) Administration during anesthesia: Administration during anesthesia may cause hypotension; in such cases, blood volume should be increased. 2. Congestive heart failure 1) Adults: Administer an initial dose of 2 mg once daily in the morning. If well tolerated, the dose may be increased to 4 mg after 2 weeks. Adjust the dose according to the patient's clinical response. In patients at high risk of symptomatic hypotension (e.g., patients with salt depletion, with or without hyponatremia; patients with volume depletion; or patients receiving potent diuretics), these risk factors should be corrected before administration of this medicine, if possible. Blood pressure, renal function, and serum potassium should be carefully monitored before and during treatment with this medicine. 2) Patients with severe congestive heart failure, elderly patients, patients with very low baseline arterial pressure, patients with renal impairment, and patients receiving high doses of diuretics: Administer an initial dose of 1 mg once daily with close and careful monitoring. Adjust the dose as appropriate according to age and symptoms. 3. Stable coronary artery disease 1) Adults: Administer 4 mg once daily for 2 weeks as the initial dose. Thereafter, if 4 mg is well tolerated, administer 8 mg once daily, taking renal function into consideration. 2) Elderly patients: Administer 2 mg once daily for 1 week as the initial dose, followed by 4 mg once daily for the next week. Thereafter, administer 8 mg once daily, taking renal function into consideration. Increase the dose to a higher dose only if the lower dose is well tolerated, and taking renal function into consideration. 3) Patients with renal impairment: Adjust the dose as follows according to renal function (creatinine clearance). 4. Patients with hepatic impairment No dose adjustment is necessary in patients with hepatic impairment.
This drug can cause a serious side effect of a significant decrease in white blood cell counts in patients with weakened immune systems, so caution is needed. The risk is particularly higher in patients with certain diseases such as systemic lupus erythematosus or scleroderma, or with impaired kidney function. Angioedema may occur during the use of this medication, where sudden swelling of the face, lips, tongue, etc., can happen, and if symptoms arise, usage must be stopped immediately and a doctor should be consulted. Patients receiving treatment for bee sting allergies may experience severe allergic reactions when taking this medication, so they must consult their doctor. Patients undergoing hemodialysis using specific dialysis membranes should inform their medical staff, as the use of this drug may be risky. Pregnant women or those who may become pregnant should avoid this drug as it can cause serious harm to the fetus and should stop immediately if pregnancy is confirmed. This drug is not recommended for certain allergy patients, those with a history of angioedema, kidney problems, specific heart conditions, or children under 18 years of age. Additionally, the risk may increase when used with potassium-sparing diuretics or lithium, so consultation with a healthcare provider is essential. When starting treatment, low doses are recommended to prevent hypotension and require careful monitoring. Side effects such as cough, dizziness, headache, nausea, rash may commonly occur, and in severe cases, blood abnormalities or liver dysfunction may happen. Patients with hypertension, heart disease, and diabetes should regularly check blood pressure, blood sugar, and kidney function while taking this drug. Interactions may occur when taken with other antihypertensive medications or specific drugs, so it is vital to inform healthcare providers of all medications being used. In case of overdose, symptoms such as hypotension, shock, or heart problems may arise, thus immediate emergency treatment is required. This drug may cause dizziness or fatigue, so caution is advised when driving or operating machinery. Store in a tightly sealed container, away from children, and do not transfer to another container.
