Treatment of depressive disorders, anxiety disorders (including panic disorder and social anxiety disorder), and obsessive-compulsive disorder
This medication is used for mental health issues such as major depressive disorder, panic disorder, social anxiety disorder, generalized anxiety disorder, and obsessive-compulsive disorder. It is especially effective in treating panic disorder whether or not it is accompanied by agoraphobia. These conditions help improve anxiety and stress-related symptoms. It assists in stabilizing patients' mental state and alleviating symptoms to enhance quality of daily life.
The safety of doses exceeding 20mg per day has not been established. This drug is administered once daily in the morning or evening, without regard to meals. Major depressive disorder: The usual dosage is 10mg once daily. Depending on the patient's response, the dose may be increased to a maximum of 20mg per day. If the dose is to be increased to 20mg, it should be increased at intervals of at least 1 week. In general, 2-4 weeks are required for antidepressant effects to appear. Even after symptoms have resolved, drug therapy for at least 6 months is required to ensure a sustained treatment effect. Panic disorder with or without agoraphobia: It is recommended to start with 5mg per day for 1 week and then increase the dose to 10mg per day. Depending on the patient's response, the dose may be increased to a maximum of 20mg per day. The maximum effect occurs after approximately 3 months. The safety and efficacy of long-term administration of this drug for more than 10 weeks in the treatment of panic disorder have not been systematically studied. The physician should periodically reassess each patient to determine the need for continued treatment. Social anxiety disorder: The usual dosage is 10mg once daily. In general, 2-4 weeks are required for improvement in symptoms. Thereafter, depending on the patient's response, the dose may be reduced to 5mg per day or increased to a maximum of 20mg per day. Social anxiety disorder is a condition with a chronic course, and continued treatment for 12 weeks is recommended to ensure a treatment effect. Long-term treatment in responding patients has been studied for 6 months and should be considered individually for each patient to prevent relapse. The usefulness of long-term treatment should be reassessed regularly. Generalized anxiety disorder: The initial dosage is 10mg once daily. Depending on the patient's response, the dose may be increased to a maximum of 20mg per day after at least 1 week of treatment. A long-term treatment study was conducted in patients who responded to 12 weeks of open-label treatment, with 20mg per day administered for at least 6 months. The physician should regularly reassess the usefulness of long-term treatment with this drug for each patient. Obsessive-compulsive disorder: The usual dosage is 10mg once daily. Depending on the patient's response, the dose may be increased to a maximum of 20mg per day. A long-term treatment study was conducted in patients who responded to 16 weeks of open-label treatment, with 10mg or 20mg per day administered for at least 24 weeks. Obsessive-compulsive disorder is a chronic condition; treatment should be continued for a sufficient period to ensure recovery from symptoms. If this drug is administered long term, the usefulness and dosage should be regularly reassessed for each patient. Patients with renal impairment: No dosage adjustment is necessary in patients with mild or moderate renal impairment. Caution should be exercised in patients with severe renal impairment (CLCR < 30㎖/min). Patients with hepatic impairment: It is recommended to administer an initial dose of 5mg per day for the first 2 weeks. Depending on the patient's response, the dose may be increased to 10mg per day. Discontinuation: If treatment with this drug is discontinued, the dose should be gradually reduced over at least 1-2 weeks to avoid the possibility of withdrawal symptoms. Switching from MAO inhibitors used to treat psychiatric disorders: When discontinuing an MAO inhibitor used to treat psychiatric disorders and starting treatment with this drug, an interval of at least 14 days must be allowed. Conversely, when starting an MAO inhibitor to treat psychiatric disorders, at least 14 days must have elapsed since discontinuing this drug. Other MAO inhibitors, such as linezolid or methylene blue: Patients receiving linezolid or intravenous methylene blue should not start treatment with this drug because of the increased risk of serotonin syndrome. For patients requiring more urgent treatment for a psychiatric condition, including hospitalization and other interventional procedures, treatment should be considered. If urgent administration of linezolid or intravenous methylene blue may be necessary in a patient already receiving this drug, and no alternative to linezolid or intravenous methylene blue is available and the benefits of treatment with linezolid or intravenous methylene blue are judged to outweigh the risk of serotonin syndrome in a particular patient, this drug may be discontinued immediately and linezolid or intravenous methylene blue administered. Patients should be monitored for symptoms of serotonin syndrome for 2 weeks after administration of linezolid or intravenous methylene blue, or until 24 hours after the last dose, whichever occurs first. Treatment with this drug may be resumed 24 hours after the last dose of linezolid or intravenous methylene blue. The risk associated with nonintravenous administration of methylene blue (oral tablets or local injection) or intravenous administration of this drug at 1mg/kg or less is unclear. Nevertheless, clinicians should be aware of the possibility of emergency symptoms of serotonin syndrome with these uses.
This medication is an antidepressant used to treat psychiatric disorders such as major depression. It should be used cautiously in children, adolescents, and young adults, as it may increase the risk of suicidal thoughts and behaviors. It is particularly not recommended for children and adolescents under 18, as safety and efficacy have not been established. This drug may be dangerous when used with certain medications, so patients taking MAO inhibitors, pimozide, or drugs that prolong the QT interval should not use it. Caution is needed for patients with glaucoma or a history of glaucoma, as this medication may dilate the pupils and increase intraocular pressure. During the first 1 to 2 weeks of treatment, abnormal reactions such as anxiety, insomnia, and restlessness may commonly occur, but these symptoms often improve over time. This medication can cause side effects such as dizziness, nausea, vomiting, headache, weight changes, and decreased libido; rarely, serious adverse reactions like cardiac arrhythmia, bleeding, or hyponatremia may occur. During treatment, thoughts or behaviors related to suicide, aggression, and symptoms of anxiety may develop, so patients and caregivers should closely monitor for any unusual signs and consult a physician immediately if they occur. Additionally, sudden cessation of treatment can provoke withdrawal symptoms such as dizziness, anxiety, and sleep disturbances, so it should be tapered off gradually under medical supervision. This medication can affect the heart, so patients with heart disease or at risk of QT interval prolongation should consult their physician before use and may need an ECG. Women who are pregnant or breastfeeding should consult healthcare professionals before deciding whether to use this drug, as it may affect the fetus or newborn. It is advisable not to take this medication with alcohol and caution is necessary when combined with other serotonergic drugs due to an increased risk of serotonin syndrome. For elderly patients, it is recommended to start with a low initial dose and adjust carefully. Caution may be required when driving or operating machinery, depending on symptoms.
