This medication is used to lower and control blood pressure in patients with essential hypertension. It also helps improve symptoms in heart failure patients who cannot use ACE inhibitors, particularly those in NYHA class II–IV. After myocardial infarction, it contributes to improved survival by reducing the risk of death in patients with impaired left ventricular function. Thus, it is effective for various heart-related conditions ranging from essential hypertension to heart failure and post-myocardial infarction management. It is used to enhance blood pressure and cardiac function tailored to the patient's condition.

How to take

Adult dose

For essential hypertension, the recommended oral dose of valsartan is 80 mg once daily. If blood pressure control is inadequate, it can be increased up to 160 mg daily, with a maximum of 320 mg. Elderly patients (75 years and older) should start with 40 mg once daily. Patients with kidney disease, volume depletion, or liver disease each require dose adjustments, and those with severe liver disease and dialysis patients should not be treated.

Precautions

This medication can cause serious harm to the fetus when taken during pregnancy, particularly in the second and third trimesters. If pregnancy is confirmed, discontinue use immediately. Women who are pregnant or may become pregnant, and breastfeeding women should not take this medication. Patients with severe liver dysfunction, cirrhosis, or biliary obstruction should avoid using this medication. Patients with hereditary angioedema or a history of angioedema should not use this medication. Patients with severely impaired kidney function, primary hyperaldosteronism, or certain kidney disorders associated with diabetes should not take this medication. Elderly patients, those with hyperkalemia, mild liver dysfunction, heart valve disease, or cardiovascular diseases should use this medication with caution. Patients with heart failure or after myocardial infarction should use this medication while closely observing changes in kidney function. This medication lowers blood pressure, and when taken for the first time, may cause a rapid drop in blood pressure leading to dizziness or fainting, so caution is needed. Patients undergoing hemodialysis or on strict sodium restriction, and those taking diuretics may be at risk for hypotension and should start at a low dose while closely monitoring their condition. This medication contains specific dye components, so patients with allergies to them should be cautious. Due to the risk of hyperkalemia, when combined with potassium-sparing diuretics or potassium supplements, blood potassium levels should be closely monitored. Taking this medication concurrently with non-steroidal anti-inflammatory drugs (NSAIDs) may reduce the blood pressure-lowering effect and increase the risk of kidney dysfunction, so caution is advised. When taken with lithium preparations, lithium levels may increase, necessitating monitoring of blood lithium levels. This medication does not significantly affect the CYP450 enzyme system, hence the risk of metabolic interactions with other drugs is low, though interactions related to certain drug transporters in the liver may occur. Safety and efficacy in children and adolescents under 18 years of age have not been established. In case of overdose, severe hypotension, decreased consciousness, or shock may occur, and immediate medical treatment is required. Store this medication out of reach of children and avoid transferring it to other containers. Caution is necessary when driving or operating machinery, as symptoms such as dizziness may occur.
