  • Poor blood sugar control
  • type 2 diabetes
View approved original text

This medication is used in the treatment of overweight adults with type 2 diabetes who find it difficult to control blood sugar levels through diet and exercise alone. It can be taken alone or in combination with other oral hypoglycemic agents or insulin for more effective blood sugar control. This helps maintain stable blood sugar levels and contributes to the prevention of diabetes-related complications.

How to take

Adult dose

The dosage should be determined based on individual drug effectiveness and tolerability, and should not exceed the recommended daily maximum dose. The usual starting dose is 500 mg of extended-release metformin hydrochloride taken once daily with the evening meal. Blood sugar levels should be measured after one week to adjust the dose. The maximum daily dose is 2000 mg. When increasing the dose, it should be done by increments of 500 mg per week.

Precautions

This medication can rarely cause serious lactic acidosis, so symptoms must be reported to healthcare professionals immediately if they appear. There is a risk of hypoglycemia when used with other diabetes medications, so caution is required. It should not be administered to patients with moderate to severe renal impairment, dehydration, or acute heart disease. Particularly, when undergoing tests that use iodinated contrast agents, this medication should be temporarily discontinued and resumed only after normal kidney function is confirmed. Patients allergic to this medication or those with type 1 diabetes or a history of lactic acidosis should not use it. In cases of severe infections or serious trauma, or around the time of surgery, this medication should be temporarily discontinued, and can be restarted once the condition stabilizes. Patients with poor nutritional status or impaired liver function, as well as those consuming excessive alcohol, should use this medication with caution. This medication may cause side effects such as gastrointestinal disturbances, vitamin B12 deficiency, and taste alteration; especially gastrointestinal symptoms may occur frequently at the beginning of treatment but gradually improve. Regular kidney function tests are necessary, and this medication should be discontinued if kidney function deteriorates. Alcohol can increase the risk of side effects, so excessive drinking should be avoided during treatment. During pregnancy, it may affect fetal growth, so consultation with a specialist is advised for those planning to become pregnant or who are pregnant. While breastfeeding, this medication is secreted into breast milk, so careful consideration should be given regarding breastfeeding and whether to discontinue the medication. Elderly patients are often prone to reduced kidney function, so dose adjustment and regular monitoring of kidney function are necessary. In cases of overdose, the medication can be removed via hemodialysis, so if an overdose is suspected, inform healthcare professionals immediately. Many drugs interact with this medication, so if other medications are being taken, be sure to inform healthcare professionals. When driving or working at heights, be aware of symptoms of hypoglycemia or lactic acidosis, and take immediate action if symptoms occur. Do not chew or crush the tablets; they should be swallowed whole, and the tablet coating may appear in the stool, which is normal.
