Relief of neuropathic pain; treatment of epilepsy (partial seizures)
This medication is used for the treatment of epilepsy. It is particularly effective for simple or complex partial seizures in adolescents aged 13 and older and adults, as well as for seizures that are secondarily generalized. It can also be used as an adjunctive therapy for partial seizures in children aged 3 years and older or adults. Additionally, it helps relieve pain caused by neuropathies. It is utilized to improve symptoms related to various neurological disorders.
Monotherapy and adjunctive therapy for epilepsy in patients aged 13 years and older: To achieve a maintenance dose of 900 mg/day of gabapentin, titrate gradually over the first 3 days of treatment as follows. Day 1: Administer one 300 mg capsule of this medicine once daily, or one 100 mg capsule of this medicine 3 times daily (gabapentin 300 mg/day) Day 2: Administer one 300 mg capsule of this medicine twice daily, or 2 capsules of this medicine 100 mg 3 times daily (gabapentin 600 mg/day) From Day 3 onward: Administer one 300 mg capsule of this medicine 3 times daily, or 3 capsules of this medicine 100 mg 3 times daily (gabapentin 900 mg/day) Alternatively, as a starting dose, one 300 mg capsule of this medicine may be administered 3 times daily (gabapentin 900 mg/day). The dose may be increased to 1,200 mg/day of gabapentin, and the total daily dose must not exceed 2,400 mg of gabapentin. (There are insufficient data on the safety and efficacy of doses exceeding 2,400 mg/day of gabapentin.) Adults with neuropathic pain (18 years and older): To achieve a maintenance dose of 900 mg/day of gabapentin, titrate gradually over the first 3 days of treatment as follows. If necessary, the dose may be increased to 1,800 mg/day of gabapentin within one week, and the total daily dose must not exceed 3,600 mg of gabapentin. When taking this medicine, the total daily dose should be divided into 3 doses. Debilitated patients: In debilitated patients with low body weight or patients after transplant surgery, the gabapentin dose should be increased in increments of 100 mg. Elderly patients: Dosage adjustment may be necessary due to age-related decline in renal function (see table). Patients with renal impairment (creatinine clearance of 80 mL/min or less) or patients undergoing hemodialysis: Dosage adjustment is required based on the following table. Renal function, creatinine clearance (mL/min) ≥80: Total daily dosea (mg/day) 900∼2,400 Renal function, creatinine clearance (mL/min) 50∼79: Total daily dosea (mg/day) 600∼1,800 Renal function, creatinine clearance (mL/min) 30∼49: Total daily dosea (mg/day) 300∼900 Renal function, creatinine clearance (mL/min) 15∼29: Total daily dosea (mg/day) 150b∼600 Renal function, creatinine clearance (mL/min) <15: Total daily dosea (mg/day) 150b∼300 a: The total daily dose should be divided into 3 doses. b: Administer 3 × 100 mg every 2 days. In patients with creatinine clearance below 15 mL/min, the dose should be reduced in proportion to creatinine clearance. Dosage in patients on hemodialysis Initial dose: 300∼400 mg of gabapentin is recommended. Maintenance dose: Administer 200∼300 mg of gabapentin after 4 hours of hemodialysis, and do not administer this medicine on days when hemodialysis is not performed. Swallow this medicine whole with sufficient fluid. It may be taken with or without food. When administered 3 times daily, take care that the interval between doses does not exceed 12 hours to prevent recurrence of seizures. If a dose of this medicine is missed (if more than 12 hours have elapsed since the last dose), the physician should decide whether an additional dose should be taken to make up for it. When used concomitantly with antacids containing magnesium or aluminum, administer this medicine at least 2 hours after the antacid. The duration of treatment with this medicine should depend on clinical need. Treatment of epilepsy generally requires long-term administration. If treatment with this medicine is to be discontinued, or if it is to be administered concomitantly with another antiepileptic drug, the change should be made gradually over at least 1 week. The safety and efficacy of treatment for neuropathic pain for more than 5 months have not been evaluated. To reduce the main adverse effects, such as somnolence, dizziness, fatigue, and ataxia, the first dose on the first day of treatment should be taken at bedtime.
Adjunctive therapy for children aged 3 years to 12 years: To achieve a maintenance dose of 30 mg/day of gabapentin per kg of body weight, titrate over the first 3 days of treatment as follows. Day 1: Administer 10 mg/kg of body weight of gabapentin Day 2: Administer 20 mg/kg of body weight of gabapentin From day 3: Administer 30 mg/kg of body weight of gabapentin The dose may be increased, if necessary, to a maximum of 40 mg per kg of body weight per day. Divide the total daily dose of this medicine into 3 doses.
This drug is used as an antiepileptic agent to control seizures. There is an increased risk of mental health issues such as suicidal thoughts, suicidal behavior, and depression during treatment, so if these symptoms arise, consult a doctor immediately. Abruptly stopping this medication can lead to worsening seizures or persistent seizure states, so it must be discontinued gradually under a doctor's instruction. Animal studies reported a potential for certain cancers, but risks in humans are unclear. There have been reports of sudden and unexpected death during treatment, which may relate to the risks of epilepsy itself. Serious side effects such as pancreatitis may occur during treatment, so if persistent upper abdominal pain, nausea, or vomiting occurs, discontinue use immediately and inform a physician. Particularly in children aged 3 to 12 years, neuropsychiatric adverse reactions such as emotional instability, aggression, decreased concentration, and hyperactivity may occur, so caution is required. This medication can increase the risk of severe sedation, respiratory depression, coma, or death when taken concurrently with central nervous system depressants like narcotics, so strict medical supervision is required when used together. Allergic reactions or severe skin reactions may occur; if symptoms such as rash, fever, or lymph node swelling arise, discontinue use immediately. There may be risks to the fetus during pregnancy, so consult a doctor if pregnant or planning to become pregnant. Nursing mothers should also exercise caution, and decisions should be made in consultation with a healthcare provider if necessary. Drowsiness or dizziness may occur; caution is advised when driving or operating machinery, and the prescribed dosage and schedule should be strictly adhered to. Patients with decreased kidney function or the elderly may require dosage adjustments, so follow a doctor's instructions. There may be weight gain during treatment, so regular weight monitoring and, if necessary, management of diet and exercise are recommended. Overdosage may lead to coma or other serious conditions; take care not to exceed the recommended dosage and seek immediate medical attention in emergency situations.
