Treatment of major depressive disorder
This medication is used for the treatment of major depressive disorder.
The therapeutic dose of this medicine should be adjusted according to the patient's clinical condition, and the lowest effective dose possible should be administered. 1. Adults The initial dose of mirtazapine is 15 mg once daily. The dose may need to be increased until an appropriate clinical effect is achieved. The usual effective daily dose is 15~45 mg of this medicine. 2. Administration in special populations 1) Patients with renal impairment Because the clearance of this medicine may be reduced in patients with moderate to severe renal impairment (creatinine clearance < 40 mL/min), caution should be exercised when prescribing it. 2) Patients with hepatic impairment Because the clearance of this medicine may be reduced in patients with hepatic impairment, caution should be exercised when prescribing it. In particular, this medicine has not been studied in patients with severe hepatic impairment; therefore, caution should be exercised when prescribing it. 3) Elderly patients The recommended dose is the same as for adults. When increasing the dose, careful monitoring is necessary to achieve a satisfactory and safe effect. 3. Switching from MAOIs used to treat psychiatric disorders When discontinuing an MAOI used to treat a psychiatric disorder and starting treatment with this medicine, an interval of at least 14 days should be allowed. Conversely, when starting an MAOI to treat a psychiatric disorder, at least 14 days should have elapsed after discontinuing this medicine. 4. Other MAOIs, such as linezolid or methylene blue Patients receiving linezolid or intravenous methylene blue should not start this medicine because of the increased risk of serotonin syndrome. For patients requiring treatment for a more urgent psychiatric condition, including hospitalization, other interventions should be considered. It may be necessary to urgently administer linezolid or intravenous methylene blue to a patient already receiving this medicine. If there are no alternatives to linezolid or intravenous methylene blue and, for a specific patient, the benefits of treatment with linezolid or intravenous methylene blue are judged to outweigh the risk of serotonin syndrome, this medicine may be discontinued immediately and linezolid or intravenous methylene blue administered. Patients should be monitored for symptoms of serotonin syndrome for 2 weeks after administration of linezolid or intravenous methylene blue, or until 24 hours after the last dose, whichever occurs first. Treatment with this medicine may be restarted 24 hours after the last dose of linezolid or intravenous methylene blue. The risk associated with administration of methylene blue by nonintravenous routes (oral tablets or local injection), or with intravenous administration of this medicine at a dose of 1mg/kg or less, is unclear. Nevertheless, clinicians should be aware of the possibility of emergency symptoms of serotonin syndrome with these uses. Because the half-life of this medicine is 20∼40 hours, once-daily administration is sufficient. If possible, take it at a consistent time in the evening before bedtime. The daily dose may also be divided into equal doses taken once each in the morning and evening. To allow sufficient time to evaluate the therapeutic response to the dose, do not change the dose at intervals of less than 1~2 weeks. With treatment at an appropriate dose, a therapeutic effect appears within 2~4 weeks. If the therapeutic effect is insufficient, the dose may be increased up to the maximum dose. If there is no effect after continuing treatment for a further 2~4 weeks, discontinue treatment. Treatment should continue for at least 4∼6 months until the patient's symptoms have completely resolved. Thereafter, gradual discontinuation is recommended to avoid withdrawal symptoms. Take this medicine orally with water; swallow it without chewing.
Children and adolescents under 18 years of age: The efficacy of this medicine has not been demonstrated in two short-term clinical trials, and due to safety concerns, it should not be used in children or adolescents under 18 years of age (see sections 4. Adverse Reactions and 8. Use in Children and Adolescents under Precautions for Use).
This drug is an antidepressant primarily used to treat symptoms associated with depression. Caution should be exercised in children, adolescents, and young adults, as there is an increased risk of suicidal thoughts or behavior. Abnormal behavioral changes such as suicidal tendencies, worsening depression symptoms, and aggression should be carefully monitored during treatment. Agranulocytosis, characterized by a decrease in white blood cell count, may rarely occur, so if symptoms of infection such as fever or sore throat appear, consult a doctor immediately. Patients with impaired liver or kidney function, heart disease, or hypotension should use this medication with particular caution. Additionally, this medication may cause drowsiness and dizziness, so activities requiring alertness such as driving or operating machinery should be approached with care. Since appetite and weight may increase, it is advisable to check weight changes periodically. Alcohol consumption should be avoided as it may enhance the central nervous system depressant effects. There may be drug interactions, so be sure to inform your doctor or pharmacist of any other medications being taken. Pregnant or nursing women should consult a doctor before taking this medication. This drug is not recommended for children and adolescents, and should be administered cautiously to the elderly. In the case of overdose, immediate medical attention should be sought, and this medication should be kept out of reach of children.
Not everyone experiences these, and they can vary from person to person.
