Treatment of epilepsy (including absence and tonic-clonic seizures) and mania associated with bipolar disorder
This medication is effectively used for several forms of epilepsy, including absence seizures (petit mal), tonic-clonic seizures (grand mal), myoclonic seizures, psychomotor seizures, and mixed seizures. It is also utilized in the treatment of manic episodes related to bipolar disorder. However, the safety and efficacy for long-term use (over 3 weeks) for mania have not been fully established, thus the continuation of treatment should be determined by re-evaluating each patient's condition. Therefore, this medication assists in treating epilepsy seizures and manic episodes in bipolar disorder.
1. Epilepsy Adults: Administer 20-30mg per kg of body weight orally in 1-2 divided doses daily. If epileptic seizures are well controlled with a daily dose of 20-30mg per kg of body weight, it may be administered once daily. Based on currently available information, when switching from enteric-coated sodium valproate tablets to this medicine, the same dose as before should be maintained. 1) Patients receiving other antiepileptic drugs: Gradually increase the dose of this medicine over 2 weeks until an appropriate dose is reached, while reducing the dose of the other antiepileptic drugs currently being used, with careful attention to seizure control. 2) Patients not receiving other antiepileptic drugs: Gradually increase the dose at intervals of 2 - 3 days over 1 week until an appropriate dose is reached. 2. Bipolar disorder The dose should be determined individually and reviewed by a physician. The daily dose should be determined according to age and body weight. Adults Initial dose: The recommended initial dose is 20 mg of sodium valproate per kg of body weight per day, administered orally in 1-2 divided doses. Increase the dose as quickly as possible to the lowest therapeutic dose that achieves the desired clinical effect. Maintenance dose: The recommended maintenance dose is 1,000 mg to 2,000 mg of sodium valproate per day. Adjust the dose according to the clinical effect in each patient to achieve a blood valproate concentration of 50-125㎍/ml. The dose should be adjusted individually according to the patient's response. The maximum daily dose is 3,000mg; do not exceed this dose. Adjust the dose as appropriate according to age, body weight, and symptoms. 3. Girls, women of childbearing potential, and pregnant women In exceptional circumstances where this medicine must be used during pregnancy in women with epilepsy, it should preferably be prescribed as monotherapy and at the lowest effective dose of a prolonged-release formulation, if possible. During pregnancy, the daily dose of a non-prolonged-release formulation should be administered in at least 2 divided doses.
Epilepsy Children (body weight 25kg or more): Administer 20-30mg per kg of body weight orally in 1-2 divided doses daily. If epileptic seizures are well controlled with a daily dose of 20-30mg per kg of body weight, it may be administered once daily.
This medication requires special caution for pregnant women and women of childbearing age. During pregnancy, there is a higher risk of congenital malformations and developmental disorders in the fetus, so it should only be administered when no other treatment options are available. Women of childbearing potential must use effective contraception and adhere strictly to a pregnancy prevention program. This medication may increase the risk of suicidal thoughts or behavior, so mental state and mood changes should be carefully monitored during treatment. It may affect liver function, requiring regular liver function tests, especially in the first 6 months after starting treatment. If symptoms of liver damage appear, medical attention should be sought immediately, and treatment should be discontinued. It should not be administered to patients suspected of mitochondrial dysfunction or urea cycle disorders due to the risk of hyperammonemia. Patients with secondary carnitine deficiency may require carnitine supplementation as symptoms can worsen with this medication. Sudden abdominal pain, nausea, vomiting, etc., can indicate the possible occurrence of life-threatening pancreatitis, so immediate consultation with medical personnel is advised. Severe skin adverse reactions or angioedema may occur, and treatment should be stopped immediately if symptoms like skin rash or edema develop. Elderly patients may experience more severe symptoms of drowsiness and dehydration, necessitating sufficient hydration and nutritional intake, and dosage adjustments may be required if symptoms are severe. Thrombocytopenia may occur, so regular blood tests are necessary, and caution is required in cases of bleeding tendency. This medication interacts with many other drugs, especially when co-administered with carbapenem antibiotics or estrogen-containing preparations. Due to central nervous system depressant effects, drowsiness or dizziness may occur, so driving or operating dangerous machinery should be avoided. Sudden changes in dosage during treatment may exacerbate seizures, so dosage adjustments should only be done slowly under medical supervision. In children, regular assessments of liver function and blood tests should be performed to detect adverse reactions early. It is very important to continuously confirm safety through regular consultations and tests with medical professionals while taking this medication.
