Summary

Main use
Treatment and suppression of genital herpes, treatment of herpes zoster, and treatment and suppression of herpes simplex in immunocompromised patients
Who uses it
Adults
Adult dosage
1. Treatment of herpes zoster Adults: Administer famciclovir orally at a dose of 250 mg per dose, 3 times daily for 7 days. It is preferable to start treatment as soon as symptoms appear after infection. 2. Treatment of initial genital herpes infection Adults: Administer this drug orally at a dose of 250 mg per dose, 3 times daily for 5 days. It is preferable to start treatment as soon as symptoms appear after infection. 3. Treatment of acute recurrent genital herpes infection Adults: Administer this drug orally at a dose of 125 mg per dose, twice daily for 5 days. It is preferable to start treatment during the prodromal period or as soon as symptoms appear after infection. 4. Suppression of recurrent genital herpes Adults: Administer this drug orally at a dose of 250 mg per dose, twice daily. Temporarily discontinue treatment periodically every 6–12 months to observe whether the natural course of the disease has changed. 5. Patients with renal impairment: Because the creatinine clearance of this drug is reduced in patients with renal impairment, dose adjustments should be made with caution. The dosage and administration for these patients are as follows. 1) Treatment of herpes zoster and initial genital herpes infection: Creatinine clearance (mL/min/1.73㎡) Dose 30–59 250 mg per dose, twice daily 10–29 250 mg per dose, once daily 2) Treatment of acute recurrent genital herpes infection: Creatinine clearance (mL/min/1.73㎡) Dose 30–59 125 mg per dose, twice daily 10–29 125 mg per dose, once daily 3) Suppression of recurrent genital herpes infection: Creatinine clearance (mL/min/1.73㎡) Dose 30–59 250 mg per dose, twice daily 10–29 125 mg per dose, twice daily 4) If only serum creatinine is known, creatinine clearance can be calculated using the following formula. Men: {body weight (kg) × (140-age)}/ {72 × serum creatinine (mg/dL)} Women: 0.85 × the value calculated for men 5) Patients with renal impairment undergoing hemodialysis For patients on hemodialysis, administration every 48 hours is preferable, taking the dialysis interval into account. Because 4 hours of hemodialysis reduces the plasma concentration of this drug by approximately 75%, administer this drug immediately after dialysis. The recommended dose is 250mg for patients with herpes zoster and 125mg for patients with genital herpes. 6. Patients with hepatic impairment: No dose adjustment is required for patients with chronic compensated liver disease. There is insufficient treatment experience in patients with overt chronic decompensated liver disease; therefore, a clear recommended dose has not been established.

Precautions

This medication should not be used in patients who have had hypersensitivity to the drug itself, its components, or to the active substance penciclovir. Additionally, this medication contains lactose, so it should not be administered to patients with genetic issues such as lactose intolerance, lactose hydrolysis enzyme deficiency, or glucose-galactose malabsorption. Special caution is required when using this medication in patients with impaired renal function, and dosage adjustment may be necessary as needed. During treatment with this medication, symptoms such as headache, dizziness, nausea, vomiting, abdominal pain, and diarrhea may occur, especially in elderly patients who may experience drowsiness or confusion more frequently. Rarely, thrombocytopenia, palpitations, liver dysfunction, skin rash or itching, and severe skin reactions have been reported, so if these symptoms occur, it is important to consult a healthcare professional immediately. This medication does not cure genital herpes; therefore, if genital herpes symptoms are present during treatment, it is important to avoid sexual contact and maintain safe sexual practices. If central nervous system symptoms such as dizziness, drowsiness, or confusion occur, driving or operating machinery should be avoided. Patients with mild to moderate liver dysfunction do not require special dosage adjustments; however, caution is needed for patients with severe liver dysfunction as drug efficacy may be reduced. This medication has a low possibility of drug interactions, but when used with drugs excreted renally, such as probenecid, blood concentration may rise, which requires monitoring for toxicity. Pregnant women or nursing mothers should only use this medication if the benefits outweigh the risks, and it is preferable not to administer it to children as safety and efficacy in this group have not been established. Elderly patients generally do not require dosage adjustments, but dosage adjustments may be necessary if renal function is impaired. In case of overdose, appropriate supportive and symptomatic treatment is required, and drug concentration may be reduced through hemodialysis. This medication should be stored in a tightly closed container to protect it from air and moisture. It is also recommended to store it in a cool and dry place below 30 degrees Celsius, out of reach of children, and avoid transferring it to another container.
