Treatment of refractory, recurrent severe arrhythmias, ventricular arrhythmias, and atrial arrhythmias
This medication is used for various arrhythmias, including atrial and ventricular arrhythmias, specifically for recurrent severe arrhythmias that do not respond to other anti-arrhythmic drugs. It helps stabilize arrhythmias when a rapid therapeutic response is needed or when oral administration is difficult. Therefore, this drug plays a crucial role in the treatment of patients with severe arrhythmias.
1. Loading regimen: Dilute 5㎎/kg of amiodarone hydrochloride for adults in 250㎖ of dextrose injection and administer slowly over 20 minutes-2 hours. The same dose may be repeated 2-3 times within 24 hours. 2. Maintenance regimen: Dilute 10-20㎎/kg in 250㎖ of dextrose injection and administer over 2-3 days (the average daily dose is 600-800㎎, and the maximum daily dose is 1200㎎.) 3. On the first day of intravenous administration, switch to oral therapy (600㎎ per day); the dose may be increased to 800-1000㎎ per day. 4. Do not use a solution containing less than 2 ampoules (300㎎) of this drug in 500㎖ of dextrose injection because it is unstable, and do not mix it with any drugs other than dextrose injection. The dose may be adjusted according to age and symptoms.
This drug poses a risk of a rapid drop in blood pressure or severe deterioration of circulatory function upon injection, thus a rapid bolus intravenous injection is not recommended. It is safer to dilute and administer slowly via infusion if possible. In emergency situations where other treatments are ineffective, it should be administered in a cardiac intensive care unit with continuous monitoring of ECG and blood pressure. It is advisable not to give this drug as a bolus intravenous injection to patients with severe hypotension or significantly weakened cardiac and respiratory function. When administering this drug, do not mix it with other medications in the syringe, and do not administer it via the same injection line as other drugs. Patients awaiting heart transplantation should consider switching to another anti-arrhythmic medication before this drug is administered. This drug may prolong the QT interval on ECG, but this is due to the drug's action and does not necessarily indicate toxicity. However, if heart conduction disorders or arrhythmias occur, administration should be discontinued. If dyspnea or dry cough occurs, there may be an inflammation of the lungs, and immediate examination is required, with early discontinuation leading to recovery. Regular liver function tests should be conducted, and if liver markers significantly elevate, the dosage may need to be reduced or discontinued. If severe rashes or blisters occur on the skin, administration should be discontinued immediately. This drug interacts with many medications, so patients must inform their doctor of any other medications they are taking, especially those that lower heart rate or cause hypokalemia. It is advisable that this drug is not administered to pregnant women, those who may become pregnant, nursing mothers, neonates, or premature infants. Additionally, this drug should be given in a special facility under cardiac monitoring, with sufficient explanation of the drug's effects and risks to the patient and family, obtaining their consent. Elderly patients should be particularly cautious due to potential weaknesses in respiratory and liver function. During administration of this drug, exposure to sunlight should be avoided, and sunscreen should be used if necessary. In case of overdose, symptom-based treatment should be conducted as the drug is not removed through dialysis, requiring careful management. Store this drug in a sealed container to prevent exposure to air or moisture, ideally in a cool place below 25 degrees Celsius. Proper storage helps maintain the drug's efficacy for a longer duration.
