Summary

Main use
Essential hypertension, adjunctive treatment of congestive heart failure, and treatment of chronic stable angina
Who takes it
Adults
Adult dose
Do not increase the dose or discontinue use without the physician's instructions. 1. Essential hypertension 1) Adults (1) For the first 2 days, administer 12.5 mg of carvedilol orally once daily, then administer 25 mg orally once daily. (2) The usual dose is 25 mg orally once daily for 2 weeks. If an increase is needed, increase to 25 mg twice daily. The maximum dose per administration is 25 mg, and the maximum daily dose is 50 mg. 2) Elderly patients In some elderly patients, 12.5 mg once daily may provide sufficient effect. However, if the effect is insufficient after 2 weeks of treatment, the dose may be increased. 2. Chronic stable angina 1) For the first 2 days, administer 12.5 mg of this drug orally twice daily, then administer 25 mg orally twice daily. 2) The usual dose is 25 mg twice daily for 2 weeks. If an increase is needed (excluding elderly patients), the dose may be increased to 50 mg twice daily. 3. Congestive heart failure Before starting treatment with this drug, the patient's existing standard treatment regimen (conventional treatment for chronic heart failure with an ACE inhibitor, a diuretic, and usually a digitalis preparation) should be stable. Fluid retention should also be minimized before starting treatment with this drug. 1) The usual initial treatment is 3.125 mg of this drug orally twice daily for 2 weeks. If the patient tolerates this treatment well, increase the dose to 6.25 mg twice daily for 2 weeks, then increase to 12.5 mg twice daily for 2 weeks, and then increase to 25 mg twice daily. 2) Increase the dose gradually after taking each dose for at least 2 weeks, up to the dose tolerated by the patient. 3) The minimum effective dose is 6.25 mg twice daily. The maximum dose is 25 mg twice daily for patients weighing 85 kg or less, and 50 mg twice daily for patients weighing more than 85 kg. 4) To control the rate of absorption, it is recommended to take the drug with meals in the morning and evening. Taking it with meals slows the rate of absorption and may reduce the incidence of orthostatic hypotension.

Precautions

This medication should not be used in certain patients. For example, patients with an allergy to this medication or its components, those in cardiogenic shock, patients with severe conduction disturbances, patients with cor pulmonale, patients with diabetic ketoacidosis, patients with severely impaired cardiac function, patients with asthma or chronic obstructive pulmonary disease, and women who are pregnant or breastfeeding may be at risk if they take this medication. Additionally, patients with diabetes, a history of severe allergies, hypotensive patients, and those with severely impaired kidney or liver function should use this medication cautiously. Since this medication can affect cardiac function, patients with heart diseases should be particularly careful. Symptoms such as dizziness, headache, and fatigue may occur while taking this medication, and in severe cases, the heart rate may slow significantly or heart failure may worsen. Additionally, adverse effects such as nausea, diarrhea, skin rash, and liver function abnormalities might arise, so any unusual symptoms should prompt immediate consultation with healthcare professionals. Regular checks of heart function, blood pressure, and liver and kidney function should be conducted for long-term users, and discontinuation should not be abrupt; instead, the dosage should be gradually reduced as directed by a physician. In particular, angina patients should be cautious as sudden discontinuation may worsen symptoms. This medication may interact with other medications, so it is essential to inform healthcare providers of any current medications. Alcohol consumption may enhance the blood pressure-lowering effect, so caution is advised. Women who are pregnant or breastfeeding should not take this medication, and safety has not been established for children and adolescents under 18 years of age; therefore, it is not recommended. In the event of an overdose, severe hypotension, slowed heart rate, and respiratory distress may occur, necessitating immediate contact with a medical institution to receive appropriate treatment. When storing this medication, it should be kept in a dark container to avoid light exposure and placed in a location out of reach of children.
