Treatment of partial-onset seizures in epilepsy; relief of neuropathic pain
This medication is used for the treatment of epilepsy. It is effective for the treatment of simple or complex partial seizures, with or without secondary generalized symptoms, as a monotherapy in adolescents aged 13 years and older and adults. Additionally, it can be used as an adjunct therapy for the same partial seizures in children aged 3 years and older through to adults. Furthermore, it is helpful in alleviating neuropathic pain, contributing to the improvement of the patient's pain. Therefore, this drug is used to effectively treat both epilepsy and neuropathic pain.
1. Epilepsy 1) Monotherapy and adjunctive therapy in patients aged 13 years and older To achieve a maintenance dose of 900 mg/day of gabapentin, titrate gradually over the first 3 days of treatment as follows. Day 1: Administer one 300 mg capsule of this medication once daily, or one 100 mg capsule of this medication 3 times daily (gabapentin 300 mg/day) Day 2: Administer one 300 mg capsule of this medication 2 times daily, or two 100 mg capsules of this medication 3 times daily (gabapentin 600 mg/day) From Day 3: Administer one 300 mg capsule of this medication 3 times daily, or three 100 mg capsules of this medication 3 times daily (gabapentin 900 mg/day) Alternatively, as a starting dose, one 300 mg capsule of this medication may be administered 3 times daily (gabapentin 900 mg/day). The dose may be increased to 1,200 mg/day of gabapentin, and the total daily dose must not exceed 2,400 mg of gabapentin. (There are insufficient data on the safety and efficacy of doses exceeding 2,400 mg/day of gabapentin.) 3) The duration of treatment should be based on clinical requirements. Long-term treatment is generally required for epilepsy. Although there is no evidence of a rebound phenomenon (increased frequency of epileptic seizures due to sudden discontinuation of treatment), sudden discontinuation of antiepileptic drugs in patients with epilepsy may cause status epilepticus. If this medication must be discontinued or administered concomitantly with another antiepileptic drug, the change should be made gradually over at least 1 week. 2. Neuropathic pain 1) Adults (18 years and older): To achieve a maintenance dose of 900 mg/day of gabapentin, titrate gradually over the first 3 days of treatment as follows. If necessary, the dose may be increased to 1,800 mg/day of gabapentin within 1 week, and the total daily dose must not exceed 3,600 mg of gabapentin. The total daily dose of this medication should be divided into 3 doses. 2) The safety and efficacy of treatment for neuropathic pain for periods longer than 5 months have not been evaluated. 3. Frail patients In frail patients with low body weight or patients after transplantation, the dose of this medication should be increased in 100 mg increments. 4. Elderly patients Dose adjustment may be necessary due to age-related decline in renal function (see table). 5. Patients with renal impairment (creatinine clearance ≤80 mL/min) or patients undergoing hemodialysis: Dose adjustment is required based on the following table. 1) Dosage for patients with renal impairment: Renal function, creatinine clearance (mL/min) ≥80: Total daily dosea 900 ~ 2,400 (mg/day) Renal function, creatinine clearance (mL/min) 50 - 79: Total daily dosea 600 ~ 1,800 (mg/day) Renal function, creatinine clearance (mL/min) 30 - 49: Total daily dosea 300 ~ 900 (mg/day) Renal function, creatinine clearance (mL/min) 15 - 29: Total daily dosea 150b~600 (mg/day) Renal function, creatinine clearance (mL/min) < 15: Total daily dosea 150b~300 (mg/day) a: The total daily dose should be divided into 3 doses. b: 3 × 100 mg should be administered every 2 days. In patients with creatinine clearance below 15 mL/min, the dose should be reduced in proportion to creatinine clearance. 2) Dosage for patients undergoing hemodialysis Initial dose: 300~400 mg of gabapentin is recommended. Maintenance dose: Administer 200~300 mg of gabapentin after 4 hours of hemodialysis, and do not administer this medication on days when hemodialysis is not performed. 6. Method and duration of administration 1) This medication should be swallowed whole with plenty of fluids and may be taken with or without food. When administered 3 times daily, care should be taken to ensure that the interval between doses does not exceed 12 hours, to prevent recurrence of seizures. 2) If a dose of this medication is missed (more than 12 hours have elapsed since the most recent dose), the physician should decide whether an additional dose should be taken to make up for it. 3) When administered concomitantly with antacids containing magnesium or aluminum, this medication should be taken at least 2 hours after the antacid. This can greatly reduce the decrease in bioavailability of this medication. 4) The duration of treatment with this medication should be based on clinical requirements. If the dose of this medication is reduced, treatment is discontinued, or the medication is changed to another alternative, this should be done gradually over at least 1 week (a longer period may be required at the prescriber's discretion). 5) To reduce the main adverse reactions, such as somnolence, dizziness, fatigue, and ataxia, the first dose on the first day of treatment should be taken at bedtime.
Adjunctive therapy in children aged 3 years to 12 years: To achieve a maintenance dose of gabapentin 30 mg/kg/day, titrate gradually over the first 3 days of treatment as follows. Day 1: administer gabapentin 10 mg/kg of body weight; Day 2: administer gabapentin 20 mg/kg of body weight; From Day 3: administer gabapentin 30 mg/kg of body weight; the dose may be increased, as needed, to a maximum of 40 mg/kg of body weight per day. The total daily dose of this drug should be divided into 3 doses.
This medication may increase the risk of suicidal thoughts or behaviors, so if mood changes or symptoms of depression occur during treatment, consultation with a physician should be done promptly. If the medication is abruptly discontinued, seizures may worsen or a status epilepticus may occur, so it should be gradually reduced according to the doctor's instructions. Long-term use may lead to weight gain, so regular weight monitoring should be conducted, and dietary control or exercise should be considered if necessary. This drug may cause drowsiness, dizziness, and motor coordination difficulties affecting the central nervous system, so it is safe to avoid driving or operating hazardous machinery at the beginning of treatment. Patients with impaired renal function should have their dosages adjusted, and elderly patients should also use caution when taking this medication. The risk of sedation and respiratory depression increases when this drug is used concurrently with narcotics or other central nervous system depressants, so it should only be combined after consulting a physician. If symptoms of acute pancreatitis (abdominal pain, nausea, vomiting, etc.) occur, the medication should be discontinued immediately and medical assistance should be sought. This drug may cause allergic reactions or serious skin reactions, so if symptoms such as rash, swelling, or difficulty breathing occur, the medication should be discontinued immediately and a hospital visit is necessary. Women who are pregnant or planning to become pregnant should consult a physician before using this medication, and caution is necessary if breastfeeding. Children, especially those aged 3 to 12 years old, should be carefully monitored for signs of emotional instability, aggressiveness, and hyperactivity, as they may experience psychiatric side effects. This medication should be taken exactly as prescribed by a doctor, and doses should not be changed or stopped arbitrarily.
