This medication provides the necessary energy and essential fatty acids in the body as part of a non-oral nutrition regimen. It helps to improve nutritional status and support bodily function. It is primarily used for patients with impaired gastrointestinal function and prevents weight loss or nutritional deficiency. This nutrient provision contributes to the overall health recovery of the patient.

How it is given

Adult administration

Generally, administer up to 2 g of soybean oil per day per kg of body weight (20 mL for 10%, 10 mL for 20%). Administration should be done via slow intravenous infusion over more than 3 hours. On the first day, for the initial injection, limit to 1 g per kg of body weight. The infusion should be adjusted according to the required amounts of essential fatty acids and calories based on the patient's physiological metabolic status.

Precautions

This medication should be used with caution, especially in premature and low-birth-weight infants. They have a reduced ability to clear fat from the body, potentially resulting in elevated levels of free fatty acids in the blood. In cases where this medication is administered intravenously to premature infants with severe respiratory diseases, there have been reported fatalities due to fat accumulation in the pulmonary vessels, necessitating extreme caution. This medication should not be used in patients with a history of thrombosis, severe liver dysfunction, severe coagulopathy, hyperlipidemia, diabetic ketoacidosis, acute shock, untreated electrolyte imbalances, unstable metabolic states, erythrophagocytic syndrome, or allergies to phospholipids. Use with caution in patients with liver dysfunction, bleeding disorders, respiratory issues, premature infants, severe bacterial sepsis, pregnant women, neurological diseases, acute pancreatitis, comatose states, and disorders of fat metabolism. Using this medication may lead to blood-related side effects such as phlebitis, vascular pain, tendency to bleed, recurrent venous embolism, and erythrocytosis. If hypersensitivity occurs, administration should be stopped immediately. Long-term use may cause liver dysfunction, so routine liver function tests should be conducted regularly, and if symptoms like hypotension, tachycardia, dyspnea, or cyanosis occur, administration should be halted and appropriate measures taken. Gastrointestinal symptoms such as nausea, vomiting, diarrhea, and thirst may also occur, along with other reported symptoms including fever, chills, facial flushing, facial swelling, chest tightness, unusual odors, and priapism. In cases of overdose, hyperlipidemia, fever, fat deposition, abnormalities in multiple organs, delayed coagulation time, thrombocytopenia, and in severe cases, coma can occur due to disorders of fat metabolism, necessitating caution. Regular monitoring of liver function, blood lipid levels, blood status, and coagulation ability is necessary with long-term use, and careful attention is needed when used with warfarin as it may reduce its effectiveness. In children, especially newborns or premature infants, it should be administered slowly due to their limited ability to process fat, and blood fat levels should be monitored while adjusting the infusion rate. Newborns with respiratory difficulties should be closely observed for signs of worsening condition. In older adults, there may be diminished physiological function, so use should be undertaken cautiously. During administration of this medication, some clinical test results may be affected, so the timing of tests should be considered. This medication is intended for slow intravenous infusion only; if administered too quickly, acute symptoms such as phlebitis, vascular pain, tachycardia, rapid breathing, nausea, vomiting, fever, chills, facial flushing, and chest tightness may occur, hence it should be infused slowly. Do not mix with other medications, avoid administration within 96 hours after the infusion of blood volume expanders, and ensure the temperature of the infusion solution is not low as this may lead to phlebitis and vascular pain. Dispose of any remaining medication after use, do not use medication that has been previously frozen, and store below 25 degrees Celsius without freezing.
